Why lower dentures move so much

An upper denture gains suction from the palate, but a lower denture sits on a narrow horseshoe-shaped ridge with the tongue and cheek muscles moving around it. After teeth are lost the bone of the ridge gradually shrinks, so the denture has less to sit on each year. The result is a denture that lifts when you open wide, rocks when you chew and lets food slip underneath, causing sore spots. Relines and adhesives can help for a time, but they do not stop the underlying problem. Anchoring the denture to the bone through implants addresses the cause of the movement directly.

How the two-implant overdenture works

Two implants are placed in the front of the lower jaw, roughly where the canine teeth used to be. This area usually has enough bone even after years of denture wear and is away from the main nerve. After healing, small attachments — commonly locator-type or ball attachments — are connected to the implants, and matching nylon or metal housings are set into the fitting surface of the denture. The denture clicks into place and is held firmly, yet you can still remove it for cleaning. Because the denture still rests partly on the gum, it is a simpler design than fixed full-arch teeth.

Can my existing denture be converted?

If your current lower denture fits reasonably well, has good tooth positions and is in sound condition, it can often be retrofitted: once the implants have healed, the attachment housings are fixed into the existing denture at the chairside. This keeps the teeth you are used to and reduces cost and visits. If the denture is old, worn, cracked or poorly fitting, a new denture is usually made and attached to the implants instead. Many patients also have a new upper denture made at the same time so that the bite is balanced. The examination determines which route makes more sense.

Who suits a two-implant lower overdenture

It is often considered for people who have no lower teeth, are frustrated with a loose lower denture, and want better stability without the larger surgery, longer treatment and more intensive cleaning that fixed full-arch teeth require. It can suit older adults well because the surgery is relatively short under local anaesthesia and the denture is easy to clean. Patients need to be medically fit for minor oral surgery, with a CBCT scan confirming bone in the front of the jaw. Those who want teeth that never come out may prefer a fixed option, which can be discussed at the same consultation.

Stages, healing and maintenance

After consultation and CBCT imaging, the two implants are placed under local anaesthesia, usually in one short appointment. The denture is relieved over the implant sites so you can keep wearing it during healing, which normally takes a few months. Attachments are then connected and the denture is converted or a new one fitted. Nylon inserts in the housings gradually wear and lose grip, so they are replaced periodically in a quick visit. Reviews every six to twelve months check the implants, gums and denture fit. Cost depends on whether a new denture is needed, and a written itemised plan is given after examination.

Modern dentures are designed for fit, bite and appearance. When a lower denture moves, two or more implants can hold it in place with click-in attachments.
