What is MARPE and can adults have it?
MARPE (miniscrew-assisted rapid palatal expansion) uses small mini-implants in the palate to widen the upper jaw itself, not just tip the teeth. It can work in many adults whose palatal suture has fused, though suitability depends on age, bone and anatomy. Dr. Pacha assesses this with a clinical exam and, usually, 3D CBCT imaging.
What is the difference between MARPE and SARPE?
Both widen a narrow upper jaw. SARPE combines expansion with jaw surgery under anaesthetic, while MARPE anchors the expander to mini-implants in the palate without a surgical cut to the jaw. For suitable adults MARPE can avoid surgery; for others SARPE remains the right choice. Dr. Pacha explains which applies to you and why.
I was told I need jaw surgery. Should I get a second opinion?
A second opinion is sensible before any jaw surgery. Some adults with a narrow upper jaw can be treated with MARPE, and some bite problems can be camouflaged with braces or TADs. Others genuinely need surgery. Dr. Pacha reviews your records, explains the options honestly and, if surgery is the right answer, coordinates with a surgeon.
Can orthodontics help snoring or mouth breathing?
Sometimes. A narrow upper jaw or retruded lower jaw can be linked to mouth breathing and snoring, and expansion or a mandibular advancement appliance may help. Snoring can also signal sleep apnoea, so Dr. Pacha recommends a sleep study where appropriate and works with sleep physicians or ENT colleagues rather than treating it alone.
Does Dr. Pacha offer Invisalign?
Yes. He plans Invisalign and other clear aligner treatment himself, including complex bites that need attachments, elastics or mini-implants alongside the aligners. Because he also offers braces, MARPE and TADs, he recommends aligners when they suit your case, not because they are the only option available.
