TMJ splint in Dubai: custom occlusal orthotics vs simple night guards

Many people with jaw pain have already tried a soft guard from a pharmacy or a basic night guard and found it only protected their teeth. A splint designed around the jaw joint is a different appliance with a different purpose.

TMJ splint in Dubai: custom occlusal orthotics vs simple night guards — Manhal Medical Center, Dubai
Quick answer

A TMJ splint is a custom occlusal orthotic made from scans of your teeth that holds the jaw in a planned, more relaxed position to reduce load on the jaw joints and calm overworked muscles. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha uses splints as a reversible first step for TMJ pain, clicking and clenching before deciding on any longer-term bite correction.

Key facts

  • A custom orthotic made from digital scans, not a generic guard
  • Aims to reduce load on the jaw joints and calm jaw muscles
  • Reversible first step for TMJ pain, clicking and clenching
  • Adjusted at follow-up visits as symptoms change
  • Orthodontic bite correction may follow in selected cases

Splint vs night guard: what is the difference?

A standard night guard is a protective layer that stops teeth wearing against each other during grinding. It is useful for tooth protection, but it does not aim to change the position of the jaw or the load on the temporomandibular joints, and soft guards can sometimes encourage more clenching. A TMJ splint, or occlusal orthotic, is made from digital scans and a carefully recorded bite. Its biting surface is shaped so that the jaw closes into a specific position chosen to decompress the joint, balance muscle activity and allow even contact. It is adjusted at follow-up visits as symptoms and jaw position change, which a simple guard is not designed to do.

Who may benefit from a TMJ splint

Splints are often considered for jaw joint pain, muscle pain in the cheeks or temples, clicking or popping, a jaw that feels stiff in the morning, clenching or grinding, and headaches linked to jaw muscle overactivity. They may also help when the bite feels “off” after joint changes. Before any appliance is made, Dr. Pacha takes a full history and examines jaw movement, joint sounds, muscle tenderness, the bite and airway factors, as night-time clenching can be associated with breathing issues during sleep. CBCT 3D imaging is used where it helps to assess the joints and bone, and MRI may be arranged through referral when the soft-tissue disc needs to be seen.

Wear schedule and what to expect

Wear depends on the diagnosis. Many patients wear the splint at night, and some also wear it for part of the day during a stabilisation phase; in those cases a separate thin daytime orthotic may be used. Patients often notice muscle tension easing and fewer headaches over the first weeks to months as the joint is unloaded, though response varies and cannot be promised. The splint is checked and adjusted at regular reviews. You may feel that your bite is different for a short time after removing it in the morning; this usually settles quickly. Clean it daily with a soft brush and cool water, and store it in its case.

When orthodontics may follow

A splint is reversible and lets the clinician see how symptoms respond to a more stable jaw position. For some patients, symptom control with the splint and self-care is all that is needed. For others, particularly when the bite does not support the comfortable jaw position, a second phase may be discussed to make the improvement more stable without the appliance. This may involve braces or aligners, expansion of a narrow upper jaw, or TADs, depending on the structural problem. Dr. Pacha only moves to this phase after symptoms are stable and the plan has been explained, including what is reversible and what is not.

Red flags and self-care

Seek urgent medical care if jaw pain comes with chest pain, arm pain or breathlessness, as heart pain can be felt in the jaw, or if you have facial swelling with fever, sudden inability to open or close the mouth, numbness or recent facial injury. Day-to-day, soft foods during flare-ups, avoiding gum chewing and very wide yawning, gentle warmth on tired muscles and noticing daytime clenching can all help. You receive a written, itemised plan after assessment; contact the clinic for his Dubai dates.

Temporomandibular disorders cause pain in the jaw joint and chewing muscles, clicking, and difficulty opening. Most cases improve with conservative, reversible care.

Questions patients ask

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.

Is a TMJ splint the same as a night guard?

No. A night guard mainly protects teeth, while a TMJ splint is designed to hold a planned jaw position and reduce joint load.

How long do I need to wear a TMJ splint?

It depends on how symptoms respond. Many people wear it for several months, with regular reviews.

Will a splint stop my jaw clicking?

It may reduce clicking or pain in some people, but clicking does not always disappear. Pain and function are the main goals.

Can I wear the splint during the day?

Some patients wear it for part of the day during stabilisation, or use a thinner daytime orthotic.

Can a splint change my bite?

It is designed to be reversible. Any lasting bite change would only come from a separate, planned phase of treatment.
