Day and night orthotics for TMJ: a paired approach to jaw pain and clicking
Many people with jaw problems clench while concentrating, driving or working at a screen, not just at night. If pain or clicking persists through the day, protecting the joint only while asleep may not be enough.

Day and night orthotics are a pair of custom appliances for TMJ problems: a thin, discreet daytime orthotic and a fuller night-time orthotic that handles the stronger clenching forces of sleep. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha uses them when symptoms continue during the day despite a night-only appliance.
Key facts
- A thin daytime orthotic paired with a fuller night orthotic
- For daytime TMJ symptoms not controlled by a night-only appliance
- Both made from digital scans and adjusted at reviews
- Phase one stabilises symptoms; phase two plans long-term care
- Daytime orthotic is usually removed for meals
Why one appliance is not always enough
Clenching and grinding during sleep can be forceful, so a night-time appliance needs to be robust and cover the teeth fully. During the day, however, many people also hold their teeth together or brace the jaw unconsciously, especially when stressed or focused, and this keeps the jaw muscles and joints loaded. A night guard or night splint protects only while it is in. For patients whose pain, muscle fatigue or clicking continues during waking hours, a paired system provides more continuous support. The daytime orthotic is designed to be thin enough for speaking and work, while the night orthotic manages the higher forces of sleep.
How the two orthotics differ
Both appliances are custom-made from digital scans of the teeth and a carefully recorded jaw position. The daytime orthotic is low-profile and usually fits over the lower teeth, so it is barely visible when talking; most people adapt to speaking with it within a few days. It is typically removed for meals. The night orthotic is fuller, often covering more of the biting surfaces, to handle stronger clenching and to hold the planned jaw position during sleep. Together they aim to keep the joint consistently decompressed and the muscles more relaxed. Both are adjusted at follow-up visits as symptoms and the jaw position change.
Phase one: stabilisation
The first phase focuses on calming symptoms and finding a comfortable, stable jaw position. Before fabrication, Dr. Pacha examines the jaw joints, muscles, range of movement and bite, considers airway and sleep factors linked to night-time clenching, and uses CBCT 3D imaging where it helps assess the joints. MRI can be arranged by referral when the joint disc needs to be evaluated. During stabilisation, the orthotics are worn as prescribed and reviewed regularly. Many patients notice reduced muscle pain and fewer headaches over the first few months, though response varies. Self-care such as soft foods during flare-ups, avoiding gum and being aware of daytime clenching supports this phase.
Phase two: the long-term plan
Once symptoms are stable, Dr. Pacha reviews whether the comfortable jaw position can be maintained without wearing appliances full time. For some patients, reducing to night-time wear and self-care is enough. Others may need a planned second phase to make the bite support the stable position, which can involve braces or aligners, expansion of a narrow upper jaw or TADs, depending on the structural cause. These steps are discussed in detail, including which parts are reversible, before anything is changed permanently. The aim is a long-term plan that suits your symptoms, bite and lifestyle rather than indefinite appliance wear by default.
When to seek help urgently and next steps
Contact a doctor urgently if jaw pain comes with chest pain, arm pain or breathlessness, if the jaw locks open or closed suddenly, or if you notice facial swelling with fever, numbness or a change in your bite after an injury. Persistent ear symptoms should also be checked medically. Dr. Moaiyad Pacha is a Consultant Orthodontist with over 15 years of experience, whose work includes TMJ, airway and sleep orthodontics in London and Dubai. He explains clearly what the orthotics can and cannot do. Prices are not published online; after assessment you receive a written, itemised plan. Contact the clinic for his Dubai dates.
Temporomandibular disorders cause pain in the jaw joint and chewing muscles, clicking, and difficulty opening. Most cases improve with conservative, reversible care.

Can I talk normally with the daytime orthotic in?
Most people adapt within a few days. It is thin and designed for speaking and work.
Do I eat with the orthotics in?
The daytime orthotic is typically removed for meals, then put back afterwards.
How is this different from a single night guard?
A night guard only protects during sleep. The paired system also supports the jaw during waking hours.
How long will I need both orthotics?
It depends on how symptoms respond. Many patients wear both during stabilisation, then reduce as advised.
Will I need orthodontic treatment afterwards?
Not always. It depends on whether the bite supports the stable jaw position once symptoms settle.
