Tooth extraction for braces — when it is needed and the alternatives
Being told you need teeth removed for braces is worrying, and online opinions are polarised. The decision should rest on measurements of your crowding, bite, lip support and bone — not on a blanket rule for or against extractions.

Teeth are removed for braces only when there is not enough space to align them healthily or when extraction is the best way to correct a bite or very prominent teeth; many patients can be treated without extractions using expansion, IPR or molar distalisation. At Manhal Medical Center in Dubai, consultant orthodontist Dr. Moaiyad Pacha assesses your space, bite, profile and airway before recommending an extraction or non-extraction plan.
Key facts
- Extractions are case-specific, not a routine step
- Alternatives: expansion, MARPE, IPR and distalisation with TADs
- Profile effect depends on lip position and tooth movement
- Airway and jaw width are part of a careful assessment
- Options are compared in a written plan
Why orthodontists sometimes recommend extractions
Extractions create space. They may be considered when crowding is severe and the teeth cannot be aligned within the available bone without pushing them too far forward or outside the gum and bone support; when the front teeth are very prominent and the lips protrude, so that retracting the teeth would improve lip closure; to compensate for a skeletal discrepancy, such as a mild Class II or III, when jaw surgery is not chosen; or when a tooth is badly damaged, impacted in an unfavourable position or has a poor long-term outlook. The teeth most often removed are premolars, but the choice depends on the individual case.
Alternatives: expansion, IPR and distalisation
Several techniques can create space without extractions. Expansion widens a narrow upper jaw: conventional expanders work in growing children, and in suitable older teenagers and adults a bone-anchored expander such as MARPE may widen the palate itself. Interproximal reduction (IPR) removes a small, controlled amount of enamel between teeth to gain fractions of a millimetre per contact. Distalisation moves molars backwards to create space at the front; bone-anchored appliances using TADs, such as a Beneslider, can move upper molars back with minimal reliance on patient compliance. Sometimes modest forward movement of the front teeth is also acceptable. Often a combination is used.
Effect on facial profile and lips
A common concern is that extractions “flatten” the face. In reality, the effect depends on how much the front teeth are moved back, the starting lip position, lip thickness and facial pattern. In a person with protrusive lips, retraction after extraction can improve lip balance; in someone whose lips are already flat or thin, retracting the teeth could be unfavourable, and a non-extraction approach may be preferred. Similarly, avoiding extractions by pushing teeth too far forward can make lips more prominent and strain the gums. A careful orthodontist plans tooth position with the profile in mind, rather than deciding by rule.
Airway and the extraction debate
Some online sources claim that extractions narrow the airway or cause sleep problems. The scientific evidence on this is mixed and does not show that well-planned extractions generally cause breathing problems, but airway and jaw width are legitimate parts of an assessment. Dr. Moaiyad Pacha, whose interests include airway-focused orthodontics, evaluates nasal breathing and the width of the upper jaw. Where a narrow jaw is the underlying cause of crowding, expansion may address both the space and the jaw width. Where extraction is clearly the better option, he explains why. The aim is a decision based on your anatomy and goals.
Deciding together at Manhal
At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha measures the crowding on a digital scan, analyses your bite, profile and lip support from photographs and X-rays, and uses CBCT where bone thickness or jaw width needs precise assessment. He then explains the extraction and non-extraction options, their likely effect on your smile and profile, the timeline and the retention needed, in a written plan. If extractions are chosen, they are coordinated with the dental team and planned around your orthodontic appliances. He holds a clinical PhD from Queen Mary University of London; contact the clinic for his Dubai dates.
It is never too late to straighten teeth. Adult orthodontics often works alongside implants, veneers or gum treatment, and discreet appliances fit professional life.

Do I need teeth removed for braces?
Not necessarily. Many patients are treated without extractions; the decision depends on crowding, bite, lip support and bone.
Which teeth are usually removed for braces?
Premolars are most commonly removed, but the choice depends on your teeth, bite and any damaged or impacted teeth.
Will extractions make my face look sunken?
Not when planned with your profile in mind. In some people, retracting protrusive teeth improves lip balance.
Can expansion avoid extractions in adults?
In suitable adults, bone-anchored expansion such as MARPE can create space; suitability depends on CBCT assessment.
Is IPR safe for my teeth?
When done in controlled amounts, IPR removes a very small amount of enamel and is a well-established technique.

Do I really need teeth extracted for braces, or are there alternatives in Dubai?
Consultant orthodontist Dr. Moaiyad Pacha at Manhal Medical Center in Dubai, assesses whether extractions are needed or whether expansion, IPR or molar distalisation could create space instead. He compares both options in a written plan.
Will removing premolars for braces change my face shape?
It depends on your lip position and how far the teeth move; Dr. Moaiyad Pacha at Manhal Medical Center, Dubai, plans tooth position with your profile in mind. In protrusive cases retraction can improve lip balance, while in flat profiles non-extraction may be preferred.
Can an orthodontist widen my jaw instead of pulling teeth?
In growing children and in suitable adults, expansion — including MARPE — can create space; Dr. Moaiyad Pacha at Manhal Medical Center in Dubai uses CBCT to check suitability. It works best when a narrow jaw is causing the crowding.
Is it true that extractions for braces can cause sleep apnea?
Current evidence does not show that well-planned extractions generally cause breathing problems, but Dr. Moaiyad Pacha at Manhal Medical Center, Dubai, includes airway and jaw width in every assessment. The decision is based on your anatomy, not a blanket rule.
