CBCT scan in orthodontics in Dubai — low-dose 3D imaging of jaws, airway and TMJ
Patients are often told they need a “3D scan” before orthodontic treatment and naturally ask why, and whether the radiation is safe. The answer depends on what needs to be measured and whether a standard X-ray can answer it.

A CBCT (cone-beam computed tomography) scan is a low-dose 3D X-ray that shows the jaws, tooth roots, airway and jaw joints in a way a flat X-ray cannot, and orthodontists use it when the answer to a clinical question depends on precise measurement. At Manhal Medical Center in Dubai, consultant orthodontist Dr. Moaiyad Pacha uses CBCT selectively — for example for MARPE planning, impacted teeth, airway and TMJ assessment — not routinely for every patient.
Key facts
- CBCT gives true 3D measurements of jaws, roots, airway and joints
- Used when it will change the plan, not routinely
- Dose is much lower than a hospital medical CT
- Essential for MARPE suture and miniscrew planning
- Findings are explained and built into a written plan
What a CBCT scan is
Cone-beam CT is a type of 3D X-ray designed for the head and face. The scanner rotates once around your head while you stand or sit still, capturing data that is reconstructed into a three-dimensional model. The scanning itself usually takes well under a minute, and nothing enters the mouth. Unlike a panoramic or cephalometric X-ray, which flattens structures onto a single image, CBCT lets the orthodontist view the jaws, tooth roots, palate, airway and jaw joints from any angle and measure them in true dimensions. The field of view can be limited to the area of interest, which helps keep the radiation dose as low as reasonably achievable.
When CBCT is justified in orthodontics
CBCT is not needed for every patient. Simple alignment cases are usually planned with clinical examination, photographs, digital scans and standard 2D X-rays. A 3D scan is considered when it is likely to change the treatment plan, for example: locating impacted or ectopic teeth such as canines and checking whether they are damaging neighbouring roots; assessing root position and bone thickness before significant tooth movement; evaluating the airway and nasal floor in patients with breathing concerns; examining the temporomandibular joints when there is pain or clicking; planning skeletal anchorage (TADs); and assessing jaw relationships where surgery may be considered. Dr. Moaiyad Pacha explains why a scan is recommended before it is taken.
Radiation in context
Every X-ray involves some radiation, so the guiding principle is to take a scan only when its benefit clearly outweighs the small risk. A dental CBCT generally delivers a much lower dose than a conventional hospital medical CT of the head, and a limited-field orthodontic CBCT is often comparable to a short series of standard dental X-rays, depending on the settings used. Using a scan only where it is justified, limiting the field of view and avoiding repeats all reduce exposure. Children are more sensitive to radiation, so extra care is taken in deciding when imaging is needed. If you are pregnant or might be, tell the clinic before any X-ray.
How CBCT plans MARPE and airway treatment
For miniscrew-assisted rapid palatal expansion (MARPE), CBCT is central to planning. It shows the maturity and shape of the mid-palatal suture, the thickness of the palatal bone and the safest positions for the miniscrews, so the appliance can be designed to engage the strongest bone. Suitability for MARPE depends on this anatomy rather than age alone. The same scan can show the nasal cavity and upper airway, helping Dr. Moaiyad Pacha discuss whether expansion is likely to contribute to nasal breathing in that particular patient, while being clear that MARPE on its own does not treat sleep apnoea. Follow-up imaging is taken only if it will guide decisions.
What happens at Manhal and what you receive
At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha — a consultant orthodontist with a clinical PhD from Queen Mary University of London and RCS orthodontic fellowship and membership — reviews whether a CBCT is needed at your consultation. If it is, the scan findings are explained to you in plain language, combined with your digital scan and clinical findings, and built into a written treatment plan. Findings outside orthodontics are flagged so you can follow them up with the appropriate doctor. Because Dr. Pacha divides his time between London and Dubai, contact the clinic for his Dubai dates.
It is never too late to straighten teeth. Adult orthodontics often works alongside implants, veneers or gum treatment, and discreet appliances fit professional life.

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.
Do I need a CBCT scan before braces or Invisalign?
Not usually for straightforward alignment. CBCT is recommended for impacted teeth, airway, TMJ, MARPE or complex skeletal cases.
Is a CBCT scan safe?
The dose is low compared with a medical CT and is justified only when the information will guide treatment. Tell the clinic if you may be pregnant.
How long does a CBCT scan take?
The scan itself typically takes well under a minute; positioning and checks add a few minutes.
Can a CBCT show my airway?
Yes, it shows the nasal cavity and upper airway, which helps orthodontic assessment, though it does not diagnose sleep apnoea on its own.
Can I get a copy of my scan?
Yes, patients can request their images and report to share with other doctors.

Who does CBCT 3D scans for orthodontic and MARPE planning in Dubai?
Consultant orthodontist Dr. Moaiyad Pacha uses CBCT 3D imaging at Manhal Medical Center in Dubai, when it will change the plan — for MARPE, impacted teeth, airway or TMJ assessment. He explains the findings and builds them into a written treatment plan.
Why does my orthodontist want a 3D CBCT scan instead of a normal X-ray?
A CBCT measures bone, roots, airway and joints in three dimensions, which flat X-rays cannot; at Manhal Medical Center in Dubai, Dr. Moaiyad Pacha recommends it only when that information is needed. Simple cases are usually planned without it.
How much radiation is in a dental CBCT compared with a hospital CT scan?
A dental CBCT generally uses far less radiation than a medical CT of the head, and Dr. Moaiyad Pacha at Manhal Medical Center, Dubai, limits the field of view to the area needed. Scans are taken only when clinically justified.
Is a CBCT scan needed to see if I’m suitable for MARPE as an adult?
Yes, MARPE suitability depends on suture and bone anatomy seen on CBCT, which Dr. Moaiyad Pacha assesses at Manhal Medical Center in Dubai. Age alone does not decide candidacy.
