MARPE for adults over 30: can the palate still be widened without surgery?
Adults often read that palatal expansion only works in children, or that anyone over 25 needs surgery. Bone-anchored expansion has changed what is possible for many adults, but honest assessment is essential, because the suture changes with age in ways that vary greatly from person to person.

Yes, many adults in their 30s, 40s and beyond can have their upper jaw widened without surgery using MARPE, because suitability depends on the anatomy of the midpalatal suture and bone seen on CBCT rather than on age alone. Success rates tend to fall as the suture becomes more interdigitated or fused, so some adults still need surgically assisted expansion. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha assesses each adult individually.
Key facts
- Suitability depends on suture and bone on CBCT, not age alone
- Many adults in their 30s and 40s can be treated without surgery
- Success becomes less predictable as the suture matures
- Adults may feel more pressure and expand more slowly
- SARPE is the alternative when MARPE is unlikely to work
Why age alone does not decide
The midpalatal suture joins the two halves of the upper jaw. In children it is open and responds easily to expansion. During adolescence and adulthood it gradually becomes more interlocked, and in some people parts of it fuse. Importantly, this process varies widely: some adults in their 30s or 40s still have sutures that can separate with sufficient force, while some younger adults do not. MARPE delivers expansion force through miniscrews into the bone, which can overcome resistance that tooth-borne expanders cannot. Dr. Pacha therefore does not rule adults in or out on age. Suitability depends on suture anatomy, bone quality and the amount of widening needed, as seen on CBCT.
CBCT assessment of suture maturity
A low-dose CBCT scan allows the suture to be examined in three dimensions, along with the thickness of the palatal bone where miniscrews would be placed, the roots of the teeth and the nasal floor. Orthodontists use the appearance of the suture on CBCT to estimate its maturity and how likely it is to separate. Bone thickness guides miniscrew length and position, and the custom MARPE design is planned so the screws engage the strongest available bone. The scan also measures the width discrepancy between the jaws, which shows whether the amount of expansion needed is realistic without surgery. Results are explained clearly in a written report, and the plan is confirmed only after an in-person consultation.
Factors that influence success
Several factors affect how likely MARPE is to work in an adult. Suture maturity and interdigitation are the most important. Bone thickness and density influence how firmly the miniscrews hold. The amount of expansion required matters: small to moderate discrepancies are more achievable than very large ones. Appliance design and miniscrew placement, planned from CBCT, play a key role, as does following the activation schedule exactly. General health also matters: smoking, uncontrolled diabetes, certain bone conditions or medications affecting bone may reduce suitability. Healthy gums are essential, because gum recession can worsen if teeth are tipped. Dr. Pacha explains your individual factors and realistic expectations before you decide.
What adults can expect
MARPE is placed under local anaesthetic, and the appliance is activated gradually on a set schedule. Adults often feel more pressure than children, in the palate, nose or cheekbones, and expansion may proceed more slowly. When the suture opens, a gap usually appears between the upper front teeth. After the target width is reached, the appliance stays in place for several months to consolidate bone, followed by braces or aligners to align the teeth and close the gap. Possible benefits include correction of crossbite, more space for crowding, a broader smile and, in some patients, improved nasal airflow, though breathing changes vary and MARPE does not treat sleep apnoea on its own.
When SARPE is needed instead
If the suture does not separate, or CBCT shows it is unlikely to, or the discrepancy is too large for MARPE, surgically assisted rapid palatal expansion (SARPE) may be recommended. In SARPE, a maxillofacial surgeon makes controlled cuts to release the bony resistance, usually under general anaesthesia in a hospital setting, and an expander is then activated. General anaesthesia is not offered at Manhal, so where SARPE or other jaw surgery is appropriate, patients are referred to a surgical team and Dr. Pacha coordinates the orthodontic side. Knowing this possibility in advance is important: MARPE is attempted when the evidence suggests a reasonable chance of success, with surgery as the alternative.
In adults the midline suture of the palate has usually fused, so traditional expanders mostly tip teeth instead of widening the jaw. MARPE anchors the expander to the palatal bone with small miniscrews, allowing skeletal expansion in many adults without surgical expansion. Published clinical studies, including randomised trials, report improvements in nasal airway dimensions and, in selected patients, in sleep-disordered breathing measures.

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.
Can MARPE work at 35 or 45?
In many adults, yes. Suitability depends on the suture and bone seen on CBCT rather than age alone.
Is there an upper age limit for MARPE?
There is no fixed age limit, but the chance of the suture separating generally falls as it matures, so assessment is individual.
What happens if MARPE doesn't open my suture?
Your orthodontist will discuss alternatives, which may include surgically assisted expansion (SARPE).
Is MARPE more uncomfortable for older adults?
Adults often feel more pressure during activation than younger patients, which usually settles between turns.
Will MARPE cause gum recession?
Bone-anchored expansion aims to widen bone rather than tip teeth, which reduces this risk. Gum health is checked before and during treatment.

Can I have MARPE at 40 in Dubai, or am I too old?
Consultant Orthodontist Dr. Moaiyad Pacha at Manhal Medical Center in Dubai assesses adults in their 40s for MARPE, because suitability depends on suture anatomy and bone on CBCT rather than age. Many adults can be treated without surgery. If the scan suggests a low chance of success, surgically assisted expansion may be discussed.
What decides whether MARPE will work for an adult over 30?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai looks mainly at suture maturity, palatal bone thickness and the size of the jaw-width discrepancy on CBCT. Health factors such as smoking and gum condition also matter. He explains your individual chances before treatment.
If MARPE fails in an adult, what happens next?
At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha discusses alternatives if the suture does not separate, which may include surgically assisted rapid palatal expansion (SARPE) with a maxillofacial surgical team. General anaesthesia is not offered at the clinic, so surgery would be arranged by referral.
Does MARPE in adults improve breathing through the nose?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai explains that MARPE may increase nasal cavity volume in some adults, which can support nasal airflow. Results vary between individuals. MARPE does not treat sleep apnoea on its own, so a sleep study is advised when apnoea is suspected.
