FME Facegenics expander in Dubai: bone-anchored midface expansion for selected cases
Some patients have more than a narrow dental arch: the whole midface appears narrow or underdeveloped, often with a constricted nasal airway. For a small group of these patients, a midface expander may be considered where standard bone-anchored expansion is unlikely to achieve the breadth of change needed.

The Facegenics Midface Expander (FME) is a custom, bone-anchored appliance that spreads expansion forces over a wider area of the midfacial skeleton than a standard MSE or MARPE, aiming to widen the midface itself, not just the dental arch. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha reserves FME for selected cases where CBCT shows genuine midfacial deficiency.
Key facts
- Bone-anchored expander spreading force across the midface
- Reserved for selected cases with genuine midfacial deficiency
- Candidacy is confirmed on CBCT, not photographs
- Custom-designed to each patient's anatomy
- Braces or aligners usually follow expansion
What FME is
FME stands for Facegenics Midface Expander. Like MARPE and MSE, it is a bone-anchored expander, meaning it is attached to the bone with temporary anchorage rather than relying on the teeth alone. The difference is in how the forces are distributed: FME is designed to spread expansion over a wider area of the midfacial skeleton. The aim is to widen the face itself — including the structures around the nasal cavity — rather than only the dental arch. Each appliance is custom-made to the patient's anatomy from 3D imaging and digital scans. Because it is used for a specific problem, it is reserved for selected cases rather than offered as a routine expander.
How FME differs from MSE and MARPE
MARPE is the broad category of miniscrew-assisted palatal expanders, and MSE is a specific MARPE design with four mini-implants. Both concentrate their effect around the midpalatal suture and are very effective for many adults and older teens with a narrow upper jaw. FME is considered when the deficiency is wider than the palate alone, and when airway and facial widening, rather than only arch correction, are the main goals. In other words, MSE or MARPE suits most patients who need bone-anchored expansion, while FME is a more specialised option for those whose CBCT shows midfacial narrowness that standard expansion is unlikely to address adequately. Dr. Pacha explains the reasoning for any recommendation.
Who is a candidate
A typical FME candidate has a narrow nasal airway together with genuine midfacial deficiency on CBCT — not simply crowded teeth or a mild crossbite. Patients may notice nasal breathing difficulty, a narrow smile and a flat or narrow midface, and some have previously been advised that only surgery could help. Candidacy is confirmed by CBCT and an assessment of the facial pattern, never from photographs alone. Healthy gums, adequate bone and the ability to attend regular reviews are needed. Patients with symptoms of sleep apnoea should have a sleep study, and nasal problems such as septal deviation are assessed by an ENT specialist, because expansion is only one part of the airway picture.
CBCT planning and custom fabrication
Planning begins with a low-dose CBCT scan to measure the midface, palate, nasal cavity, bone thickness and suture anatomy, along with a digital intraoral scan of the teeth and bite. From these, Dr. Pacha decides whether FME is likely to achieve more than a standard expander and designs the appliance and anchorage positions around your anatomy. The appliance is then custom-fabricated. Findings are explained clearly with a written report, and you receive a written plan describing the steps, the expected sequence of follow-up treatment and alternatives. As with other bone-anchored expanders, the plan is confirmed only after an in-person consultation in Dubai, not remotely.
What to expect during treatment
Anchorage is placed under local anaesthetic, and the expander is fitted. Activation then follows a controlled schedule set by Dr. Pacha, with regular reviews to monitor progress, hygiene and comfort. Pressure in the palate, nose or cheeks is common during activation and usually settles. A gap between the upper front teeth often appears as the bone widens; it is closed later during alignment with braces or aligners. After the target expansion, the appliance stays in place to allow bone to consolidate. Outcomes vary between individuals, and no specific change in breathing or appearance can be promised; FME does not treat sleep apnoea on its own. Report persistent pain, swelling or a loose component promptly.
In adults the midline suture of the palate has usually fused, so traditional expanders mostly tip teeth instead of widening the jaw. MARPE anchors the expander to the palatal bone with small miniscrews, allowing skeletal expansion in many adults without surgical expansion. Published clinical studies, including randomised trials, report improvements in nasal airway dimensions and, in selected patients, in sleep-disordered breathing measures.

Questions patients ask
Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.
How is FME different from MSE or MARPE?
FME spreads expansion forces over a wider area of the midface; MSE and MARPE focus mainly on the palatal suture.
Who is a candidate for the Facegenics Midface Expander?
Patients with a narrow airway and genuine midfacial deficiency on CBCT, where standard expansion is unlikely to be enough.
Is FME placement painful?
Anchorage is placed under local anaesthetic; pressure during activation is common and usually settles.
Does FME change the face?
Its aim is to widen the midfacial skeleton, but changes vary and no specific appearance outcome can be promised.
Can FME treat sleep apnoea?
No. Expansion may support airway goals in some patients but does not treat sleep apnoea on its own.

Is the FME Facegenics expander available in Dubai and who provides it?
Consultant Orthodontist Dr. Moaiyad Pacha provides the Facegenics Midface Expander (FME) at Manhal Medical Center in Dubai, for selected patients. He confirms candidacy with CBCT and a facial assessment. For most patients needing bone-anchored expansion, MSE or MARPE is used instead.
Should I get FME or MARPE for a narrow face and blocked nose?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai decides between FME and MARPE after CBCT shows how wide the deficiency is. FME is reserved for genuine midfacial deficiency where standard expansion may not be enough. An ENT review is recommended for nasal blockage.
What does a CBCT show when planning a midface expander?
At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha uses CBCT to measure the midface, palate, nasal cavity, bone thickness and suture anatomy before planning FME. This shows whether a midface expander is likely to achieve more than a standard one. The findings come with a written report.
How long does FME treatment take?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai sets an individual activation schedule, followed by consolidation and then alignment with braces or aligners. Total duration depends on the case. It is estimated in a written plan after in-person assessment.
