MSE expander in Dubai: bone-anchored upper-jaw expansion for older teens and adults
Many adults with a narrow upper jaw, crossbite or crowding have been told that only jaw surgery can widen the palate after the teenage years. Bone-anchored expanders such as MSE have changed that conversation for many patients, although they are not right for everyone.

The Maxillary Skeletal Expander (MSE) is a bone-anchored palatal expander held by four small titanium mini-implants, so the widening force goes into the bone rather than tipping the teeth. It is used for older teenagers and adults whose palatal suture has matured. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha assesses suitability with a CBCT scan before recommending MSE.
Key facts
- Four mini-implants anchor the expander to palatal bone
- Designed for older teens and adults with a matured suture
- Suitability is confirmed on a CBCT scan
- Placement is done under local anaesthetic
- A temporary gap between the front teeth is expected
What MSE is and how it differs from tooth-borne expanders
A conventional palatal expander is cemented to the back teeth, so the force passes through the teeth to the bone. In children, whose midpalatal suture is open, this widens the jaw. Once the suture matures, typically from the mid-to-late teens onwards, the same force mainly tips the teeth outwards instead of separating the bone, which can strain the gums and relapse. MSE is a specific expander design with four mini-implants placed into the palatal bone, so the force is delivered directly to the skeleton. This allows bony widening of the upper jaw in many patients beyond the age when tooth-borne expanders work, offering a non-surgical alternative for some who were previously told they needed surgery.
Who may suit MSE
MSE is considered for older teenagers and adults with a transverse deficiency — an upper jaw that is too narrow for the lower jaw. Signs can include a posterior crossbite, crowding, a narrow smile with dark corners, and sometimes nasal breathing difficulties linked with a narrow nasal floor. Some patients are referred because they have been told they need surgically assisted expansion. Suitability depends on bone thickness, the shape and maturity of the suture and the amount of expansion needed, which is why it is confirmed on a CBCT scan, not on a clinical examination alone. Healthy gums and good oral hygiene are also important, and patients with certain bone conditions may not be suitable.
Planning with CBCT
A low-dose CBCT (3D) scan shows the thickness of the palatal bone where mini-implants can be placed, the anatomy of the midpalatal suture, the position of roots and the nasal floor. Dr. Pacha uses this information, along with a digital intraoral scan, to decide whether MSE is likely to work, where the implants should go and how much expansion is needed. CBCT also helps identify cases where bone-anchored expansion is unlikely to separate the suture, so patients can be counselled honestly about alternatives such as surgically assisted expansion. Findings are explained in plain language and summarised in a written report, and the plan is confirmed only after an in-person consultation.
The process: placement and activation
The appliance is made to fit your mouth from the digital scan. At the fitting appointment, the four mini-implants are placed through the gum into the palatal bone under local anaesthetic, and the expander is attached to them and to the back teeth. You then turn the central screw following a set schedule, gradually widening the upper jaw over several weeks until the target expansion is reached. Mild soreness or pressure during the first days of activation is common and usually settles. A gap often appears between the upper front teeth as the bone separates, which is an expected sign that the suture is opening and is closed later with braces or aligners.
Timeline and what comes next
After active expansion, the appliance stays in place for several months to let new bone consolidate in the widened suture. It is then removed, and orthodontic alignment with braces or aligners usually follows to close the front gap and finish the bite. Total treatment time varies with the case and is estimated in your written plan. Expected benefits can include correction of crossbite, more room for crowded teeth, a broader smile and, in some patients, improved nasal airflow, although breathing changes vary and MSE is not a treatment for sleep apnoea on its own. Risks include discomfort, gum irritation and, uncommonly, implant loosening or uneven expansion, which are monitored at each review.
In adults the midline suture of the palate has usually fused, so traditional expanders mostly tip teeth instead of widening the jaw. MARPE anchors the expander to the palatal bone with small miniscrews, allowing skeletal expansion in many adults without surgical expansion. Published clinical studies, including randomised trials, report improvements in nasal airway dimensions and, in selected patients, in sleep-disordered breathing measures.

Do I need surgery to widen my upper jaw as an adult?
Not necessarily. Many adults can be treated with MSE without surgery, but suitability must be confirmed on CBCT.
Is MSE placement painful?
Mini-implants are placed under local anaesthetic. Mild soreness during the first days of activation is common.
How long does MSE stay in?
Active expansion takes several weeks, then the appliance usually stays for several months while the bone consolidates.
Will a gap appear between my front teeth?
Yes, usually. It shows the suture is opening and is closed later with braces or aligners.
Does MSE fix sleep apnoea?
No. It may improve nasal airflow in some patients but does not treat sleep apnoea on its own.

Where can I get an MSE expander as an adult in Dubai?
Consultant Orthodontist Dr. Moaiyad Pacha offers the Maxillary Skeletal Expander (MSE) at Manhal Medical Center in Dubai, for older teens and adults with a narrow upper jaw. Suitability is confirmed with a CBCT scan and in-person examination. Braces or aligners usually follow expansion.
How is MSE different from a normal palate expander?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai explains that MSE is anchored to the palatal bone with four mini-implants, while normal expanders push on the teeth. That allows bony widening after the suture has matured. Tooth-borne expanders mainly work in growing children.
I'm 25 and was told I need jaw surgery to widen my palate — could MSE work instead?
Possibly — Dr. Moaiyad Pacha at Manhal Medical Center in Dubai assesses many adults in their 20s for MSE as a non-surgical option. A CBCT scan shows whether the bone and suture are likely to respond. If not, surgically assisted expansion may still be advised.
What is the timeline for MSE treatment from start to finish?
At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha typically plans several weeks of activation, several months of consolidation, then alignment with braces or aligners. Total duration depends on the case and is set out in a written plan. Retainers follow at the end.
