MARPE for sleep apnea and snoring — what maxillary expansion can and cannot do
People who snore or have been diagnosed with sleep apnoea often read that jaw expansion can “cure” it. The truth is more measured: expansion can be one part of care for selected patients with a narrow upper jaw, but realistic expectations and proper diagnosis are essential.

MARPE (miniscrew-assisted rapid palatal expansion) widens a narrow upper jaw and nasal floor in suitable adults, which may improve nasal airflow in some people, but it does not treat or cure obstructive sleep apnoea on its own. At Manhal Medical Center in Dubai, consultant orthodontist Dr. Moaiyad Pacha assesses whether MARPE could play a role, always alongside a sleep study and the advice of a sleep physician, and sometimes combined with a mandibular advancement device (MAD).
Key facts
- MARPE widens a narrow upper jaw and nasal floor in suitable adults
- It may improve nasal airflow but does not cure sleep apnoea alone
- A sleep study is needed first to diagnose and grade OSA
- Often combined with a MAD; CPAP remains standard for severe OSA
- Plans are coordinated with sleep physicians and ENT specialists
How a narrow upper jaw relates to breathing
The upper jaw forms the floor of the nose, so a narrow, high-arched palate is often associated with a narrower nasal cavity. Reduced nasal airflow can encourage mouth breathing, and a narrow upper arch may leave less room for the tongue, which can sit lower and further back. These factors may contribute to snoring and to upper airway collapse during sleep in some people. Obstructive sleep apnoea, however, has many contributors, including body weight, neck size, soft-tissue anatomy, the position of the lower jaw, nasal problems, alcohol, sedatives and sleep position. A narrow upper jaw is only one possible piece of the picture.
What MARPE does — and what the evidence suggests
MARPE uses an expander anchored to the palate with miniscrews, planned from a CBCT scan, to separate the mid-palatal suture in adults whose suture has matured beyond the reach of conventional tooth-borne expanders. Widening the upper jaw also widens the nasal floor. Studies show that expansion can increase nasal cavity volume and, in some patients, improve nasal airflow and subjective breathing, and some report reductions in snoring or apnoea indices in selected cases. Results vary considerably between individuals, and expansion is not a stand-alone cure. MARPE is primarily an orthodontic treatment for transverse (width) problems; any airway benefit is a potential bonus, not a guaranteed outcome.
Why a sleep study comes first
Snoring alone does not tell you whether you have sleep apnoea or how severe it is. A sleep study — either a validated home study or an overnight study in a sleep laboratory — measures breathing pauses, oxygen levels and other parameters, giving an apnoea–hypopnoea index that grades the condition as mild, moderate or severe. This baseline is essential before any appliance or expansion is considered, and for judging whether treatment has helped. Dr. Pacha works with sleep physicians and ENT specialists, who can also look for nasal obstruction, enlarged tonsils or other causes. Severe sleep apnoea needs management led by a sleep physician.
Combining expansion with a MAD or CPAP
For many adults, MARPE would be part of a broader plan. A custom mandibular advancement device (MAD), worn at night, holds the lower jaw slightly forward to keep the airway more open and is a recognised option for snoring and mild-to-moderate obstructive sleep apnoea, especially for people who cannot tolerate CPAP. CPAP remains the standard treatment for moderate to severe sleep apnoea. Expansion may be considered alongside these when there is a genuine transverse deficiency, sometimes to create room for a MAD or to improve nasal breathing that makes CPAP easier. Any change to sleep apnoea therapy should be made with your sleep physician and confirmed with follow-up testing.
Red flags and assessment at Manhal
Seek medical advice promptly if you have loud snoring with witnessed pauses in breathing, choking or gasping at night, excessive daytime sleepiness — particularly if you have dozed while driving — morning headaches, or high blood pressure that is hard to control, as untreated sleep apnoea affects health and safety. At Manhal Medical Center in Dubai, consultant orthodontist Dr. Moaiyad Pacha, whose practice focuses on airway, sleep and TMJ orthodontics, assesses your jaw width, bite and airway with CBCT and explains, in a written plan, whether MARPE, a MAD or other options are appropriate. Contact the clinic for his Dubai dates.
The size and position of the jaws influence the space available for the tongue and airway. Airway-focused orthodontics assesses breathing, tongue posture and sleep alongside the teeth, and plans treatment that supports all of them.

Can MARPE cure my sleep apnoea?
No. MARPE may improve nasal airflow in some patients, but it does not treat or cure obstructive sleep apnoea on its own.
Do I need a sleep study before MARPE for snoring?
Yes. A sleep study confirms whether you have sleep apnoea and how severe it is, and provides a baseline.
Can I stop using CPAP after MARPE?
Only your sleep physician can advise this, based on follow-up testing; do not stop CPAP on your own.
Who is suitable for MARPE?
Adults with a genuinely narrow upper jaw whose suture and bone anatomy on CBCT allow skeletal expansion.
Is a MAD better than MARPE for snoring?
They work differently; a MAD repositions the lower jaw during sleep, while MARPE widens the upper jaw. Some patients use both.
