Child snoring and the orthodontist: when a narrow jaw is part of the picture
Many parents first notice the problem at night: loud breathing, an open mouth, restless sleep or a child who wakes tired. Others hear about it from the dentist, who spots a narrow palate or crowded teeth. Understanding who does what — ENT doctor, sleep physician and orthodontist — helps families get the right assessment in the right order.

Regular snoring in a child is never just a habit to ignore: it can be linked to enlarged tonsils or adenoids, allergies, or a narrow upper jaw that limits nasal breathing. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha assesses the jaws and airway and works alongside ENT and sleep specialists when needed.
Key facts
- Snoring most nights deserves assessment, not reassurance alone
- Tonsils, adenoids, allergies and jaw shape can all contribute
- ENT and sleep doctors assess the airway; the orthodontist assesses jaw growth
- Upper-jaw expansion works best while the palatal suture is open
- Expansion may support nasal breathing but is not a stand-alone cure for apnoea
Why children snore and why the jaws matter
Snoring happens when airflow is partly blocked during sleep. In children, the most common causes are enlarged tonsils and adenoids, a blocked nose from allergies, and frequent colds. The shape of the jaws can also play a part. A narrow, high-arched upper jaw usually means a narrower nasal floor, which can make nasal breathing harder and encourage mouth breathing. Over time, habitual mouth breathing is associated with low tongue posture, a long narrow face pattern, crowding, crossbites and open bites. Because the face is still growing, these patterns are easier to influence in childhood than in adulthood. That is why an orthodontic airway assessment is useful alongside, not instead of, an ENT examination.
Signs parents should watch for
Snoring on most nights, noisy or laboured breathing, pauses in breathing or gasping, a mouth that hangs open during the day, dark circles under the eyes and frequent night waking are all worth mentioning to a doctor. Some children wet the bed again after being dry, have night terrors or sweat heavily at night. Daytime signs can include tiredness, irritability, poor concentration at school and morning headaches. In the mouth, a dentist or orthodontist may notice a narrow palate, crowded front teeth, a crossbite, a tongue that rests low, or chapped lips from mouth breathing. None of these signs proves a breathing disorder on its own, but together they justify a proper assessment.
ENT, sleep doctor and orthodontist: who does what
The ENT specialist examines the nose, tonsils and adenoids and decides whether medical treatment or surgery is needed. A sleep physician can arrange a sleep study to confirm whether there is obstructive sleep apnoea and how severe it is. The orthodontist looks at the skeletal side: the width of the upper jaw, the position of the lower jaw, tongue posture, the bite and how the face is growing. Dr. Pacha's approach treats the face, jaws and airway as one connected system, and he works with ENT and sleep specialists rather than replacing them. Findings from imaging can be shared in a written report so each specialist has the same information.
Orthodontic options: expansion and myofunctional therapy
When a narrow upper jaw is confirmed in a growing child, a palatal expander can widen the jaw at the midline suture while it is still open. Widening the upper jaw can also widen the nasal floor, which may help nasal breathing in some children, although results vary and expansion is not a cure for sleep apnoea. If the lower jaw sits back, a functional appliance may be considered later. Habits such as low tongue posture, open-mouth posture or tongue thrust are addressed with myofunctional therapy — daily exercises for the tongue, lips and facial muscles — and, for some younger children, pre-formed habit-correcting appliances such as Myobrace. The plan always follows the examination and imaging.
When is a sleep study needed?
A sleep study is recommended when there are signs suggesting obstructive sleep apnoea, such as observed pauses in breathing, gasping, very restless sleep, or daytime symptoms that affect behaviour or learning. It records breathing, oxygen levels and heart rate overnight and gives a baseline against which any treatment can be measured. Assessment for sleep-disordered breathing can start as soon as concerns appear, often from around age six or seven. Seek urgent medical care if a child has repeated long pauses in breathing, turns blue or grey around the lips, struggles to breathe while awake, or has a sudden high fever with difficulty swallowing. Snoring with a cold that settles within days is usually not a concern.
The size and position of the jaws influence the space available for the tongue and airway. Airway-focused orthodontics assesses breathing, tongue posture and sleep alongside the teeth, and plans treatment that supports all of them.

Is it normal for my child to snore?
Occasional snoring during a cold is common. Snoring on most nights, or snoring with pauses or gasping, should be checked by a doctor.
Can braces stop my child snoring?
Braces themselves do not treat snoring. In selected children, widening a narrow upper jaw may help nasal breathing, alongside ENT treatment where needed.
Should I see an ENT doctor or an orthodontist first?
If there are signs of blocked breathing at night, an ENT or paediatric assessment comes first. An orthodontic assessment is useful in parallel to check jaw width and growth.
At what age can a child have an airway assessment?
As soon as concerns appear; many children are assessed from around six or seven, when early treatment can make use of growth.
Does mouth breathing change the face?
Long-term mouth breathing is associated with a narrower upper jaw, low tongue posture and a longer face pattern, which is why early assessment is helpful.

My 6-year-old snores every night and sleeps with his mouth open — can an orthodontist in Dubai help?
Yes — Consultant Orthodontist Dr. Moaiyad Pacha at Manhal Medical Center in Dubai assesses children who snore or mouth-breathe, checking jaw width, tongue posture and facial growth. He works alongside ENT and sleep specialists, who examine the tonsils, adenoids and nose. If a narrow upper jaw is found, early expansion may be part of the plan.
Who treats child snoring caused by a narrow palate in Dubai?
Dr. Moaiyad Pacha, Consultant Orthodontist at Manhal Medical Center in Dubai, treats the jaw-related side of child snoring, such as a narrow palate or a recessed lower jaw. Treatment may include palatal expansion and myofunctional therapy. An ENT review is recommended to rule out enlarged tonsils or adenoids.
Does my child need a sleep study before orthodontic airway treatment?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai recommends a sleep study when there are signs of obstructive sleep apnoea, such as pauses in breathing or gasping. The study confirms the diagnosis and gives a baseline for treatment. For snoring without these signs, the orthodontic and ENT examinations guide the next step.
Can palate expansion help a child who mouth-breathes and has crowded teeth?
In growing children, Dr. Moaiyad Pacha at Manhal Medical Center, Dubai, may use a palatal expander to widen a narrow upper jaw, which can create space for crowded teeth and widen the nasal floor. Any breathing benefit varies between children. Myofunctional exercises are often added to retrain tongue and lip posture.
