Braces gum problems: swollen gums, recession and loose teeth during treatment
Swollen gums, a little bleeding when brushing or teeth that feel slightly loose can be worrying in the middle of treatment. Most of these changes are manageable with good cleaning and timely check-ups, but some need prompt attention.

Gum problems during braces are usually caused by plaque collecting around brackets, which makes gums swollen, red or bleeding. At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha checks gum health before and during orthodontic treatment, works with the clinic’s general dentist for cleaning, and screens adults for gum disease before teeth are moved.
Key facts
- Swollen, bleeding gums are usually caused by plaque around brackets
- Slight tooth looseness is normal while teeth move
- Recession risk rises where gum and bone are thin
- Adults are screened for gum disease before treatment
- Interdental brushes and gum-line brushing protect the gums
Why gums react to braces
Brackets, wires and bands create many small corners where plaque can collect, and it is harder to clean along the gum line. Plaque bacteria irritate the gums, causing gingivitis: redness, puffiness and bleeding when brushing. In some patients, especially teenagers, the gums can also overgrow slightly over the brackets, a reaction known as gingival enlargement, which usually improves with better cleaning and after braces are removed. Hormonal changes in puberty and pregnancy, mouth breathing that dries the gums, and some medicines can make the reaction stronger. Gingivitis is reversible, but if it is left untreated in susceptible people it can progress to periodontitis, which damages the bone supporting the teeth.
Recession and thin gums
Gum recession means the gum line moves down and exposes more of the tooth or root. During orthodontics, recession risk is higher where the gum and underlying bone are thin, often on the lower front teeth, and where teeth are moved outwards beyond the bony housing, such as when crowding is resolved by excessive arch widening. Hard brushing and gum inflammation add to the risk. An orthodontist reduces this by planning movements within the bone, assessed on examination and CBCT 3D imaging where indicated, and by considering skeletal expansion or space from mini-implants rather than tipping teeth outwards. Where gum thickness is a concern, the clinic can arrange assessment for gum care or grafting before or after treatment.
Teeth that feel loose
Teeth feeling slightly loose during braces is normal. To move a tooth, the ligament and bone around the root remodel, so a small amount of mobility is part of the process, and it settles once treatment ends and retainers hold the teeth. Mobility that increases markedly, a tooth that is painful to bite on, gum swelling with pus or a bad taste, or a tooth that feels much looser than its neighbours should be checked promptly. These can indicate gum infection, excess force or a problem with a particular root. Adults with a history of gum disease may have reduced bone support and need gentler forces and closer monitoring throughout treatment.
Cleaning routine that protects your gums
Brush at least twice daily with a fluoride toothpaste, angling the brush towards the gum line above and below each bracket; a soft or electric brush with an orthodontic head is helpful. Use interdental brushes to clean under the wire and between brackets, and floss with a threader or tape designed for braces. A water flosser can be a useful addition but not a replacement. Rinse after meals when you cannot brush, limit sugary snacks and fizzy drinks, and keep up professional cleaning with the general dentist during treatment. The clinic’s general dental team can show you techniques, and at each orthodontic visit Dr. Pacha also checks the gums and your cleaning.
Gum screening for adults and when to call
Before adult orthodontic treatment, Dr. Moaiyad Pacha checks the gums and bone levels and may ask for a gum assessment and cleaning first, because active gum disease must be controlled before teeth are moved. Many adults with stable, treated gum disease can still have orthodontics with careful planning. Contact the clinic promptly if you notice gums that bleed heavily or persistently, swelling with pain or pus, a tooth becoming increasingly loose, or recession appearing quickly. Seek urgent care for facial swelling, fever or difficulty swallowing. Manhal Medical Center in Dubai, is open daily and can be reached by phone or WhatsApp.
Fixed braces use brackets and wires to move teeth with high control. They remain one of the most predictable options for complex crowding, rotations and bite correction.

Why are my gums swollen with braces?
Usually plaque collecting around the brackets causes gingivitis. Better brushing and interdental cleaning often improve it within a couple of weeks.
Is it normal for teeth to feel loose with braces?
A little looseness is normal as bone remodels. Marked or increasing looseness, pain or pus should be checked.
Can braces cause gum recession?
They can contribute where gum and bone are thin or teeth are moved outside the bone. Careful planning lowers the risk.
Can I have braces if I have gum disease?
Active gum disease must be treated first. Many adults with stable gums can then have orthodontics with close monitoring.
Will swollen gums go away after braces come off?
Usually yes, especially with good hygiene, though occasionally overgrown gum needs a minor procedure.

My gums are swollen and bleeding with braces — is this normal and who can check it in Dubai?
Swollen, bleeding gums during braces are common and usually caused by plaque, and Consultant Orthodontist Dr. Moaiyad Pacha at Manhal Medical Center in Dubai checks gum health at each visit with the clinic’s general dental team. Improved cleaning often helps within weeks, but persistent bleeding or pus should be seen promptly.
Can adults with a history of gum disease still get braces in Dubai?

Why do my teeth feel loose in braces and when should I worry?
At Manhal Medical Center in Dubai, Dr. Moaiyad Pacha explains that slight looseness is normal because bone remodels as teeth move. Contact the clinic if a tooth becomes much looser, painful to bite on or the gum swells with pus.
How can an orthodontist reduce the risk of gum recession during treatment?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai plans tooth movements within the bone using examination and CBCT imaging where indicated, and may prefer skeletal expansion or mini-implants over tipping teeth outwards. Gum assessment can be arranged where tissue is thin.
