IPR teeth stripping: how interproximal reduction creates space in orthodontics
Patients often see 'IPR' on their aligner plan and worry that their teeth will be filed down or weakened. Understanding why space is needed, how little enamel is involved and what the other options are helps make an informed decision.

IPR (interproximal reduction), often called teeth stripping, means carefully polishing away a very small amount of enamel between selected teeth to create space for aligners or braces to straighten crowding. It is planned precisely, done within safe limits and usually causes little or no discomfort. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha uses IPR only where it suits the case and explains the alternatives.
Key facts
- IPR polishes a small amount of enamel between selected teeth
- Creates space for mild to moderate crowding
- Amounts are planned per contact within safe limits
- Usually comfortable; enamel has no nerves
- Alternatives include expansion, distalisation or extraction
What IPR is and why space is needed
Crowding means the teeth need more room than the arch currently provides. Orthodontists can create space in a few ways: widening the arch, moving back teeth backwards, removing a tooth, or reducing the width of some teeth slightly with IPR. IPR involves polishing the contact points between teeth using fine abrasive strips, thin discs or burs, removing a fraction of a millimetre of enamel from the sides of selected teeth. Because enamel is thickest at the sides of many teeth, a small reduction can be done safely. IPR can also reshape teeth that are triangular in form, reducing the small dark triangles that sometimes appear between front teeth once they are straightened.
How much enamel is removed
The amount is planned for each tooth surface and is small — typically a fraction of a millimetre per contact, within established safe limits for that type of tooth. The total space gained from IPR across several teeth can relieve mild to moderate crowding without needing extractions. In aligner treatment, the planned amount for each contact is shown in the treatment plan, and it may be done in stages at different appointments as teeth move into better positions. Measuring gauges are used to check the space created. Dr. Pacha considers enamel thickness, tooth shape and gum health before planning IPR, and avoids it where enamel is thin or there are large fillings at the contact.
Safety, sensitivity and long-term effects
When performed within recommended limits, research has generally not linked IPR with an increased risk of tooth decay or gum disease, provided the surfaces are left smooth and oral hygiene is good. Afterwards, the polished surfaces may be treated with fluoride to support the enamel. Some patients notice mild temporary sensitivity to cold for a few days, but many feel nothing. Small gaps may be visible immediately after IPR; these are expected and close as the teeth move. Careful flossing helps keep the contacts clean. As with any procedure, IPR is not suitable for everyone, and it is planned only after a clinical examination and X-rays where needed.
Does IPR hurt?
Most people find IPR comfortable. Enamel has no nerves, so polishing it is not painful in itself, and local anaesthetic is not usually needed. You may feel vibration, pressure from the strip or disc, or a slightly odd sensation as the teeth are separated. Protective guards are used to shield the gums and lips. Each contact takes only a short time, and the whole session is usually quick. If teeth are tight, a small separator may sometimes be placed first. Afterwards your teeth may feel slightly different to your tongue, and a little cold sensitivity may occur for a few days. Contact the clinic if sensitivity is strong or lasts longer than a week.
Alternatives: expansion, distalisation and extraction
IPR is one of several ways to create space, and the right choice depends on how much space is needed, the shape of the arches and the face, and gum health. Expansion widens a narrow arch; in growing children a palatal expander can widen the upper jaw, and in adults bone-anchored expanders such as MARPE are used in suitable cases. Moving back teeth backwards with miniscrew-supported appliances can create space without extractions in selected patients. For severe crowding or marked protrusion, removing teeth may still be the most stable option. Dr. Pacha explains the pros and cons of each approach in your written plan, so IPR is used where it helps rather than by default.
Invisalign straightens teeth with a series of removable, nearly invisible aligners made from a 3D scan. You can take them out to eat and clean, and progress is planned digitally from the first visit.

Does IPR weaken teeth?
When done within recommended limits and left smooth, IPR has not generally been linked to weaker teeth or more decay.
Does teeth stripping hurt?
Usually not. Enamel has no nerves; you may feel vibration or pressure and occasional mild cold sensitivity afterwards.
Will the gaps from IPR stay?
No. Small gaps are expected right after IPR and close as the teeth move into position.
Can IPR be avoided?
Sometimes. Expansion, moving back teeth or extraction are alternatives, depending on the case.
Is IPR used with braces as well as aligners?
Yes, IPR can be used with fixed braces or aligners when it suits the plan.
