MARPE vs SARPE: comparing miniscrew expansion and surgically assisted expansion
Adults with a narrow upper jaw are often told about two options with similar-sounding names. Understanding how they differ — and why one may be suggested over the other — helps patients ask the right questions and make an informed choice.

MARPE widens the upper jaw without surgery, using miniscrews in the palate under local anaesthetic, while SARPE combines an expander with surgical cuts to the upper jaw, usually under general anaesthesia in hospital. MARPE is less invasive with a quicker recovery; SARPE is used when the suture is too mature or the discrepancy too large for MARPE. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha uses CBCT to advise which is realistic.
Key facts
- MARPE: miniscrews, local anaesthetic, no jaw incisions
- SARPE: surgical cuts, usually under general anaesthesia in hospital
- MARPE recovery is usually quicker
- SARPE is used when the suture is too mature or expansion too large
- CBCT guides which option is realistic
What MARPE is
MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion. A custom expander is anchored to the palatal bone with two to four temporary miniscrews, often also attached to the back teeth, and planned from a CBCT scan so the screws engage the strongest bone. Turning the expander screw on a set schedule delivers force directly to the bone, aiming to separate the midpalatal suture and widen the upper jaw itself. It is placed in the clinic under local anaesthetic, with no incisions to the jaw. Many late adolescents and adults can be treated this way, because suitability depends on suture and bone anatomy on CBCT rather than age alone.
What SARPE is
SARPE stands for Surgically Assisted Rapid Palatal Expansion. An expander is fitted to the upper jaw, and a maxillofacial surgeon performs controlled cuts (osteotomies) in the upper jaw to release the bony areas that resist widening. After a short healing period, the expander is activated to widen the jaw. SARPE is usually done under general anaesthesia in a hospital setting and involves a recovery period with swelling and some time off work or study. It is a well-established procedure and remains the appropriate option when bone-anchored expansion alone is unlikely to succeed, such as when the suture is heavily interdigitated or fused, or when very large expansion is required.
Invasiveness, anaesthesia and recovery compared
MARPE involves placing miniscrews through the gum into bone under local anaesthetic, so most patients return to normal activities within a day or two, with soreness for a few days and pressure during activation. SARPE involves surgical cuts to the upper jaw, typically under general anaesthesia, so recovery includes facial swelling and bruising, a soft diet and usually more time off, with the usual risks of surgery and anaesthesia. Both are followed by a consolidation period with the expander in place, then orthodontic alignment to close the front gap and finish the bite. General anaesthesia is not offered at Manhal, so SARPE, when needed, is arranged through referral to a surgical team.
Suitability by age and anatomy
In children, conventional tooth-borne expanders work because the suture is open. From late adolescence onwards, MARPE is often the first option considered, and many adults in their 20s, 30s, 40s and beyond can be treated successfully. The chance of the suture separating generally falls as it matures, and this varies between individuals, so age alone is not decisive. CBCT shows suture morphology, bone thickness and the size of the jaw-width discrepancy. Where these suggest a reasonable chance of success, MARPE is usually tried first. Where they suggest MARPE is unlikely to work, or the expansion needed is very large, SARPE may be recommended from the outset.
How the choice is made
Dr. Pacha starts with a full examination, digital intraoral scan and CBCT. He explains the findings in a written report, including the transverse discrepancy, the appearance of the suture, bone thickness and gum condition, and outlines the realistic options: MARPE, SARPE, or in some cases orthodontic compensation without skeletal expansion. Patient priorities matter too, including attitude to surgery, time off and overall goals. If MARPE is chosen and the suture does not open, SARPE remains available as a next step. Neither option treats sleep apnoea on its own; where apnoea is suspected, a sleep study and a sleep physician's input are part of the plan.
In adults the midline suture of the palate has usually fused, so traditional expanders mostly tip teeth instead of widening the jaw. MARPE anchors the expander to the palatal bone with small miniscrews, allowing skeletal expansion in many adults without surgical expansion. Published clinical studies, including randomised trials, report improvements in nasal airway dimensions and, in selected patients, in sleep-disordered breathing measures.

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.
Which is less invasive, MARPE or SARPE?
MARPE, because it uses miniscrews under local anaesthetic without cutting the jaw.
Is SARPE done under general anaesthesia?
Usually yes, in a hospital setting. General anaesthesia is not offered at Manhal, so SARPE is arranged by referral.
Can I try MARPE before considering SARPE?
Often yes, if CBCT suggests a reasonable chance of success. SARPE remains an option if the suture does not open.
How long is recovery after SARPE?
It typically involves swelling and a soft diet for a period after surgery; the surgical team advises on time off.
Is MARPE always successful in adults?
No. Success depends on suture and bone anatomy, which is why CBCT assessment comes first.

Should I have MARPE or SARPE to widen my upper jaw at 32?
Consultant Orthodontist Dr. Moaiyad Pacha at Manhal Medical Center in Dubai uses CBCT to judge whether MARPE is likely to work for you or whether SARPE is more realistic. Many adults in their 30s can be treated with MARPE without surgery. If the suture looks too mature, SARPE may be advised.
What is the difference between MARPE and SARPE recovery?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai explains that MARPE is placed under local anaesthetic and most people resume normal activities within a day or two. SARPE involves jaw surgery, usually under general anaesthesia, with swelling and more time off. Both need a consolidation period and braces or aligners afterwards.
Does Manhal Medical Center in Dubai perform SARPE?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai provides MARPE in the clinic, while SARPE, which usually needs general anaesthesia, is arranged by referral to a surgical team. He coordinates the orthodontic side before and after surgery. General anaesthesia is not offered at the clinic.
If MARPE doesn't work, can I still have SARPE later?
Yes — Dr. Moaiyad Pacha at Manhal Medical Center in Dubai explains that SARPE remains an option if the suture does not separate with MARPE. This is discussed before treatment starts. CBCT helps estimate the chances in advance.
