Diagnostics: examination, scans and CBCT

MARPE planning starts with a comprehensive consultation: a clinical examination of the teeth, gums, bite and jaw joints, a digital intraoral scan, and a low-dose CBCT scan. The CBCT is essential, because it shows suture anatomy, palatal bone thickness, root positions and the size of the jaw-width discrepancy, which determine whether MARPE is suitable and how the appliance should be designed. A written radiology report explaining the findings is part of good practice. When comparing quotes, check whether 3D imaging and reporting are included, because a plan made without CBCT is not equivalent to one designed around it.

Custom appliance design and miniscrews

A MARPE appliance is designed for your anatomy, with two to four miniscrews positioned to engage the strongest available bone. The design work, laboratory fabrication, the miniscrews themselves and the fitting appointment under local anaesthetic all form part of the fee. Some patients need a specific design, such as the four-implant MSE or, in selected cases with genuine midfacial deficiency, a midface expander, which can change the complexity of planning and fabrication. Activation, reviews and consolidation

After fitting, there is an active expansion phase lasting a few weeks, with reviews to check that the suture is opening symmetrically and the screws remain stable, followed by several months of consolidation with the appliance in place. The number and frequency of reviews, any progress imaging, and removal of the appliance at the end all contribute to the overall plan. If adjustments or repairs are needed, the written plan should explain how they are handled. Follow-up braces or aligners and retention

MARPE widens the upper jaw, but the teeth still need aligning afterwards: the front gap that appears during expansion must be closed and the bite finished. This second phase, with fixed braces or clear aligners, is often a substantial part of the total plan, and its length depends on crowding, bite correction and complexity. Some patients also need temporary anchorage devices or other auxiliaries. Finally, retainers are needed to keep the result. When comparing costs, look at whether a quote covers only the expander or the complete treatment through to retention, as this is one of the most common reasons quotes appear very different.

Why there is no price before examination

Under UAE health advertising rules, and because each case differs, Manhal does not publish MARPE prices or offers. Instead, after your consultation and CBCT, you receive a written, itemised plan setting out each phase, what is included, the expected duration, the alternatives considered — such as conventional orthodontics or surgically assisted expansion — and how payment is structured. Ask about insurance: orthodontic cover varies widely between policies, and some plans exclude adult orthodontics, so check your policy and request pre-approval where relevant. The aim is that you can make a clear decision based on the complete picture, not a headline figure.

In adults the midline suture of the palate has usually fused, so traditional expanders mostly tip teeth instead of widening the jaw. MARPE anchors the expander to the palatal bone with small miniscrews, allowing skeletal expansion in many adults without surgical expansion. Published clinical studies, including randomised trials, report improvements in nasal airway dimensions and, in selected patients, in sleep-disordered breathing measures.
