Narrow palate treatment: signs of a narrow or high-arched palate and expansion options by age
A narrow palate is often noticed indirectly: a dentist mentions a crossbite, teeth look crowded, the smile seems narrow with dark corners, or a child breathes through the mouth. Understanding the signs and the options at different ages helps patients and parents decide when to seek an assessment.

A narrow or high-arched palate is treated by widening the upper jaw: with a conventional expander in growing children, and with bone-anchored expanders such as MARPE or MSE in older teens and adults, followed by braces or aligners. At Manhal Medical Center in Dubai, Consultant Orthodontist Dr. Moaiyad Pacha diagnoses palatal width with a clinical examination, digital scan and CBCT, and plans treatment by age and anatomy.
Key facts
- Signs include crossbite, crowding and a narrow smile
- A narrow palate often means a narrower nasal floor
- Children: conventional expanders while the suture is open
- Teens and adults: bone-anchored MARPE or MSE
- Assessment uses examination, digital scan and CBCT
Signs of a narrow or high-arched palate
In a narrow upper jaw, the roof of the mouth is often deep and V-shaped rather than broad and U-shaped. Common signs include a posterior crossbite, where some upper back teeth bite inside the lower teeth; crowding of the upper teeth, especially the canines; and a narrow smile with dark spaces, or 'buccal corridors', at the corners. Some people have a lower jaw that shifts sideways when closing. Others notice the tongue has little room and rests low, sometimes with scalloped edges. In children, mouth breathing, snoring and a long, narrow face pattern can accompany a narrow palate. None of these signs alone confirms the diagnosis, but together they are a good reason for assessment.
Why palate width matters: bite, crowding and breathing
The upper jaw needs to be wide enough for the upper teeth to fit around the lower teeth. When it is too narrow, the bite can be unstable, teeth may wear unevenly, and crowding is more likely. Because the floor of the nose is the roof of the mouth, a narrow upper jaw usually means a narrower nasal floor, which can make nasal breathing harder in some people. A narrow palate also gives the tongue less room, which may encourage low tongue posture. Widening the upper jaw can improve the bite and create space, and in some patients may support nasal airflow, although breathing benefits vary and expansion is not a treatment for sleep apnoea on its own.
Expansion options in children
In growing children, the midpalatal suture is open, so a conventional palatal expander cemented to the upper back teeth can widen the jaw at the bone. A parent turns the screw gradually on a written schedule, and the appliance stays for several months after expansion to hold the result. This is often done before or around early adolescence. A first orthodontic check around age seven helps identify narrow palates early. Expansion may be combined with myofunctional therapy or habit appliances when mouth breathing or tongue habits are present, and with ENT assessment if the nose is blocked. Some children later need braces to align the teeth fully.
Expansion options in older teens and adults
Once the suture has matured, typically from the mid-to-late teens, conventional expanders mainly tip the teeth rather than widening bone. For these patients, bone-anchored expanders are used: MARPE designs with two to four miniscrews, the four-implant MSE, or, in selected cases with genuine midfacial deficiency, the Facegenics Midface Expander. They are placed under local anaesthetic and activated over several weeks, then left to consolidate before braces or aligners. Suitability depends on suture and bone anatomy seen on CBCT rather than age alone, and many adults in their 30s, 40s and beyond can be treated. When the suture is unlikely to open, surgically assisted expansion is discussed.
Assessment and when to seek care
Dr. Pacha's assessment includes a clinical examination of the teeth, bite, jaw joints, tongue posture and breathing, a digital intraoral scan and, where indicated, a low-dose CBCT scan to measure palatal width, suture maturity, bone thickness and the airway. Findings are explained in a written report that can be shared with your GP or ENT specialist, followed by a written plan with options and timelines. Book an assessment if you or your child has a crossbite, a jaw that shifts on closing, crowding, chronic mouth breathing or a narrow smile. Seek medical care promptly for loud snoring with pauses in breathing or heavy daytime sleepiness, which may indicate sleep apnoea needing medical evaluation.
In adults the midline suture of the palate has usually fused, so traditional expanders mostly tip teeth instead of widening the jaw. MARPE anchors the expander to the palatal bone with small miniscrews, allowing skeletal expansion in many adults without surgical expansion. Published clinical studies, including randomised trials, report improvements in nasal airway dimensions and, in selected patients, in sleep-disordered breathing measures.

How do I know if I have a narrow palate?
Signs include a crossbite, crowded upper teeth, a deep V-shaped palate and a narrow smile. An orthodontic examination confirms it.
Can a narrow palate be fixed in adults?
Often yes, with bone-anchored expanders such as MARPE or MSE, after CBCT confirms suitability.
Does a narrow palate affect breathing?
It can be associated with a narrower nasal floor and mouth breathing, though breathing also depends on the nose and other factors.
What age is best to treat a narrow palate?
In childhood, while the suture is open, treatment is simplest; adults can still be treated with bone-anchored expansion.
Will braces alone widen a narrow palate?
Braces can tip teeth outwards but do not widen the bone; skeletal expansion is needed for true widening.

Who treats a narrow palate in adults in Dubai?
Consultant Orthodontist Dr. Moaiyad Pacha at Manhal Medical Center in Dubai, treats adults with narrow palates using bone-anchored expanders such as MARPE and MSE. Suitability is confirmed with CBCT. Braces or aligners follow expansion.
My child has a high-arched palate and breathes through the mouth — what should we do?
Book an assessment with Dr. Moaiyad Pacha at Manhal Medical Center in Dubai, who checks palatal width, tongue posture and breathing and may recommend a palatal expander while the suture is open. An ENT review is advised for nasal blockage. Myofunctional exercises are often added.
Can a narrow palate cause crowded teeth and a crossbite?
Yes — Dr. Moaiyad Pacha at Manhal Medical Center in Dubai explains that a narrow upper jaw often leaves too little room for teeth and lets upper back teeth bite inside the lower ones. Expansion can correct the width and create space. Braces then align the teeth.
Is narrow palate treatment different for children and adults?
Dr. Moaiyad Pacha at Manhal Medical Center in Dubai uses conventional tooth-borne expanders in growing children and bone-anchored expanders such as MARPE or MSE in older teens and adults. The difference reflects whether the palatal suture is still open. CBCT helps decide in borderline cases.
