Silver filling replacement — when to replace amalgam fillings and tooth-coloured options

Many adults in Dubai have dark fillings placed years ago and wonder whether they should swap them for white ones. The decision should be based on the condition of each filling and tooth, as well as your own preferences, rather than a blanket rule.

Silver filling replacement — Manhal Medical Center, Dubai
Quick answer

Old silver (amalgam) fillings usually need replacing when they are cracked, leaking, have decay underneath or have caused the tooth to crack, or when you prefer a tooth-coloured look; intact, healthy amalgam fillings do not automatically need removal. At Manhal Medical Center in Dubai, Dr. Fariba Alnoory examines your fillings with X-rays and explains which, if any, should be replaced and with what.

Key facts

  • Intact amalgam fillings do not automatically need removal
  • Decay, leakage, fractures and cracks are clinical reasons to replace
  • Careful removal uses water spray, suction and isolation
  • Composite, inlays, onlays or crowns are tooth-coloured options
  • Elective removal is usually postponed during pregnancy

What amalgam fillings are

Dental amalgam is a mixture of metals, including mercury bound with silver, tin and copper, that has been used for over a century to fill back teeth. It is durable and tolerant of moisture, which is why many people still have amalgam fillings that have lasted for decades. Major health and dental organisations generally consider intact amalgam fillings safe for most people, while many countries are phasing down its use for environmental reasons. Today most new fillings are tooth-coloured composite, but an existing amalgam filling that is sound and causing no problems is not necessarily something that must be removed.

When should a silver filling be replaced?

Replacement is usually recommended when there is a clinical reason. Common reasons include new decay around or under the filling, a gap or ditch at the edges where bacteria can enter, a fractured filling, or cracks in the surrounding tooth, which can happen because large amalgam fillings do not bond to the tooth and may act like a wedge over time. Sensitivity when biting or to cold can also point to a problem. Cosmetic reasons, such as dark fillings visible when you laugh, are another valid reason if you prefer a tooth-coloured appearance. Your dentist checks each filling individually, often with X-rays.

How old fillings are removed carefully

Removing amalgam produces fine particles and vapour, so dentists use careful techniques to minimise exposure. These typically include plenty of water spray and high-volume suction, cutting the filling into sections rather than grinding it all away, and isolating the tooth, sometimes with a rubber dam, where suitable. After removal, any decay is cleaned out and the remaining tooth is checked for cracks. Elective removal of sound amalgam fillings is generally postponed during pregnancy as a precaution. Your dentist can explain what to expect and answer questions about the procedure before you start.

Tooth-coloured replacement options

For small and medium cavities, composite resin is bonded to the tooth and shaped to match its colour and anatomy. Because it bonds, it can help support the remaining tooth structure. When a large old filling has left thin walls or cracks, a ceramic inlay or onlay may be stronger, as it is made to fit and then bonded, protecting weak cusps. If too little tooth remains, a crown may be advised. The right choice depends on the size of the cavity, your bite, any grinding habits and your priorities. Replacing several fillings can be staged over multiple visits.

Silver filling replacement in Dubai at Manhal

At Manhal Medical Center in Dubai, Dr. Fariba Alnoory, a general dentist with more than 14 years of experience, examines each existing filling, takes X-rays where needed and explains which fillings show signs of decay, leakage or cracking and which are sound. If replacement is appropriate, she discusses composite and other tooth-coloured options and provides a written, itemised plan. Patients who are anxious about dental work are welcome to discuss their concerns first, and several replacements can be spread over more than one visit. Consultations are available in English and Arabic.

Composite fillings repair cavities with resin matched to your tooth colour. They bond to the tooth, allowing a more conservative preparation than old metal fillings.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
