How the two concepts work

Both treatments anchor a fixed bridge of 10–14 teeth to implants in one jaw. In All-on-4, two implants are placed upright at the front and two at the back are tilted, which lets longer implants engage denser bone and avoid the sinus in the upper jaw or the nerve in the lower jaw. This tilt often reduces the need for bone grafting. In All-on-6, six implants are spread around the arch, typically more upright, giving additional support points. In both concepts, a fixed temporary bridge can often be attached soon after surgery if the implants are stable enough, and a stronger final bridge follows once healing is complete.

Bone requirements

Bone volume is usually the deciding factor. All-on-4 was designed for jaws that have lost bone at the back, where placing six implants would need sinus lifts or block grafts. All-on-6 suits patients with enough bone along more of the arch, or those willing to have grafting to create it. A CBCT 3D scan measures bone height, width and density at each site and shows the sinus and nerve positions, so the plan is based on measurement rather than preference. In some patients the scan shows five implants, or a mix of approaches, is the most sensible design.

Load distribution and bite forces

More implants mean the biting load is shared across more points, which can reduce stress on each implant and on the bridge. This can be helpful for people with strong bites, a history of clenching or grinding, a long arch, or softer bone, particularly in the upper jaw. It also provides a margin of safety: if one implant ever has a problem, a six-implant bridge may still be supported while it is managed. However, well-planned All-on-4 bridges have a long track record for many patients. The decision is about matching the design to your jaw and habits, not simply “more is better”.

Cost factors and longevity

A six-implant plan uses more implants and components, so it typically involves more surgical time and material. All-on-4 may avoid grafting, which can balance the comparison when the alternative would be All-on-6 plus sinus lifts. Other cost drivers include imaging, extractions, any grafting, the temporary bridge, and the final bridge material (zirconia or a titanium-reinforced hybrid). Longevity depends far more on hygiene, regular professional maintenance, bite control (often a night guard for grinders), smoking and general health than on implant count alone.
