All-on-4 recovery timeline — what to expect week by week

Knowing what recovery looks like makes full-arch implant treatment far less daunting. Most people return to normal daily life within days, but the implants need several months of gentle treatment to fuse with the bone.

All-on-4 recovery timeline — Manhal Medical Center, Dubai
Quick answer

After All-on-4 surgery, swelling and soreness usually peak in the first two to three days, a soft diet is followed for roughly the first two to three months while implants integrate, and the final bridge is fitted once healing is complete. At Manhal Medical Center in Dubai, consultant implantologist Dr. Manhal Helfawee gives a personalised recovery plan and reviews you at each stage.

Key facts

  • Swelling usually peaks at day 2–3
  • Soft diet for roughly the first 2–3 months
  • Speech usually adapts within 1–2 weeks
  • Final bridge after implants integrate
  • Daily cleaning under the bridge is essential

The first 72 hours

On the day of surgery you will usually leave with a fixed temporary bridge if implant stability allowed it. Expect numbness for a few hours, then soreness, some swelling of the cheeks or lips, and possibly bruising. Swelling usually peaks around day two or three. Cold packs in the first 24 hours, keeping your head raised when resting, taking prescribed or recommended medicines on schedule and plenty of fluids all help. Avoid smoking, alcohol, spitting forcefully and hot drinks on the first day. Many people take two to three days off work. A short review visit checks healing, bite and comfort of the temporary bridge.

Weeks 1 to 12: the soft diet phase

The implants are integrating with the bone (osseointegration) during the first months, so they must not be overloaded. You will usually be advised to eat only very soft food for the first week or two — soups, yoghurt, eggs, mashed vegetables, soft fish — then food you can cut with a fork for the following weeks. Avoid hard, crunchy, sticky or very chewy foods such as nuts, crusty bread, steak and ice, and do not bite into food with your front teeth. Cut food into small pieces and chew evenly on both sides. The exact timeline depends on your bone, implant stability and Dr. Manhal’s advice.

Speech and getting used to your new teeth

A new bridge changes the shape of the inside of your mouth, so some people notice a slight lisp, extra saliva or feel their tongue “searching” for space in the first days. This usually settles within one to two weeks as the tongue and lips adapt. Reading aloud, speaking slowly and repeating difficult words helps. If your bite feels high, or the bridge rubs or catches your cheek or tongue, tell the clinic — small adjustments to the temporary bridge are normal. Feedback during this phase is valuable because it guides the shape, length and contour of your final bridge.

Cleaning and hygiene during recovery

Good hygiene protects the healing gums and, long term, the implants. In the first days, gentle salt-water or prescribed rinses are typically used, and a soft brush on the bridge avoiding the surgical areas. As healing progresses you will be shown how to clean under and around the bridge with a soft or single-tufted brush, interdental brushes, floss threaders or a water flosser. Plaque left around implants can cause inflammation of the gum and, over time, bone loss around the implant (peri-implantitis), so daily cleaning and regular professional maintenance are essential. Smoking strongly increases implant complications and slows healing.

From temporary to final bridge — and warning signs

Once integration is confirmed, usually a few months after surgery, Dr. Manhal Helfawee takes new records and digital scans to make the final bridge in zirconia or a titanium-reinforced hybrid. After it is fitted, you return to a broad normal diet, with sensible care for very hard foods, and attend maintenance visits — commonly every six months. Contact Manhal Medical Center urgently if you have bleeding that does not stop with pressure, swelling that keeps increasing after day three, fever, pus, a bad taste, a loose or broken bridge, or numbness that persists. Call +971 4 328 4560 or WhatsApp +971 52 987 2411.

All-on-4 and All-on-6 replace a full arch of teeth with a fixed bridge anchored on four or six implants. The angled placement of the back implants makes use of the strongest available bone, which often avoids extensive grafting.

How painful is All-on-4 recovery?

Most people describe soreness rather than severe pain for a few days, managed with recommended pain relief; it varies with extractions and grafting.

When can I go back to work?

Many people return after two to three days, depending on the extent of surgery and the type of work.

When can I eat normally?

After the final bridge is fitted and integration is confirmed; until then a soft diet protects the implants.

Can I smoke after All-on-4?

Smoking slows healing and increases the risk of implant failure; stopping, at least during healing, is strongly advised.

How often do I need check-ups afterwards?

Usually every six months, with professional cleaning and implant checks, or as Dr. Manhal advises.

Dr. Manhal Helfawee at Manhal Medical Center, Dubai usually advises very soft foods like soups, yoghurt and mashed vegetables at first, then fork-tender food for the following weeks. Hard, crunchy and sticky foods are avoided until the final bridge is fitted.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
