Wisdom tooth close to the nerve — CBCT assessment, coronectomy and managing numbness
Being told that your wisdom tooth is “near the nerve” can be worrying, especially after reading about numbness of the lip or tongue. Careful imaging and planning reduce the risk, and understanding the options helps you make an informed decision.

When a lower wisdom tooth lies close to the inferior alveolar nerve, a 3D CBCT scan shows the exact relationship so the surgeon can plan the safest approach, which may be careful full removal or, in selected cases, a coronectomy that leaves the roots in place. At Manhal Medical Center in Dubai, Dr. Moataz Al-Khan, specialist oral and maxillofacial surgeon, assesses nerve proximity and explains the options and risks before surgery.
Key facts
- The inferior alveolar and lingual nerves are most relevant
- CBCT shows the 3D relationship between roots and nerve canal
- Coronectomy may be considered when roots touch the nerve
- Numbness lasting beyond about 24 hours should be reported
- Altered sensation often improves gradually over weeks to months
Which nerves are involved?
Two nerves are most relevant to lower wisdom tooth surgery. The inferior alveolar nerve runs through a canal in the lower jaw beneath the tooth roots and gives feeling to the lower lip, chin, lower teeth and gums on that side. The lingual nerve runs on the tongue side of the jaw near the wisdom tooth and gives sensation and taste to the side of the tongue. If either nerve is bruised or stretched during surgery, temporary numbness, tingling or altered sensation can occur. Upper wisdom teeth are not near these nerves, but may be close to the maxillary sinus.
How CBCT assesses the risk
A standard panoramic X-ray can suggest that the roots are close to the nerve canal, showing signs such as darkening of the roots, narrowing or diversion of the canal. When these signs are present, a cone beam CT (CBCT) scan provides a three-dimensional view. It shows whether the canal passes beside, between or in direct contact with the roots, how thin the surrounding bone is and the shape of the roots. This allows the surgeon to estimate the risk more accurately, choose the surgical approach, and decide whether a coronectomy might be a safer alternative for that tooth.
Full removal vs coronectomy
In most cases, careful full removal is appropriate, sometimes sectioning the tooth into pieces to reduce pressure on the nerve. When the roots are intimately related to the nerve, a coronectomy may be considered: the crown of the tooth is removed, and the roots are deliberately left in the bone below the gum to avoid disturbing the nerve. The roots usually heal over with bone. Coronectomy is only suitable for teeth without infection in the roots or significant decay, and occasionally roots migrate upwards and need a second, usually simpler, procedure. Your surgeon explains the pros and cons for your situation.
Numbness of lip, chin or tongue after removal
Some temporary numbness is expected while the anaesthetic wears off over several hours. If numbness or tingling of the lower lip, chin or tongue persists beyond about 24 hours, contact the clinic so the surgeon can document the area affected and arrange follow-up. In many cases, nerve-related altered sensation improves gradually over weeks to months as the nerve recovers, although recovery can be incomplete in a small number of cases. Management may include monitoring, protecting the lip from accidental biting or burns, and referral for further assessment if needed. Wisdom teeth do not cause numbness of the head; such symptoms need medical evaluation.
Wisdom tooth near the nerve — assessment at Manhal in Dubai
At Manhal Medical Center in Dubai, Dr. Moataz Al-Khan, a specialist oral and maxillofacial surgeon and lecturer in oral surgery, assesses lower wisdom teeth with panoramic X-rays and 3D CBCT imaging when the roots appear close to the nerve. He explains whether removal is needed, the risks for your particular tooth, whether full removal or coronectomy is appropriate, and the recovery to expect. Surgery is performed under local anaesthesia; general anaesthesia is not offered at the clinic. You receive written aftercare and follow-up arrangements, in English or Arabic.
Wisdom teeth that lack space can become impacted, trapping food and causing infection, pain, or damage to the neighbouring molar. Surgical removal is planned with X-rays to protect nearby nerves.

Is it dangerous to remove a wisdom tooth close to the nerve?
There is a higher risk of temporary or, less commonly, lasting numbness. CBCT planning and careful technique help reduce that risk.
What is a coronectomy?
Removing the crown of the wisdom tooth while leaving the roots in place to avoid disturbing the nerve.
How is lip numbness after wisdom tooth removal treated?
It is monitored and documented; many cases improve gradually over weeks to months, with referral if needed.
How is tongue numbness after wisdom tooth removal treated?
Similarly, it is assessed and followed up; lingual nerve sensation often recovers gradually.
Can a wisdom tooth cause numbness in the head?
No. Wisdom teeth can cause referred pain or headaches, but head numbness needs medical evaluation.

My dentist says my lower wisdom tooth is very close to the nerve — what are my options in Dubai?
At Manhal Medical Center in Dubai, Dr. Moataz Al-Khan assesses the tooth with a 3D CBCT scan to see its exact relationship with the nerve. Depending on the findings, he may recommend careful full removal or, in selected cases, a coronectomy that leaves the roots in place.
How long does lip and chin numbness last after wisdom tooth extraction near the nerve?
Dr. Moataz Al-Khan at Manhal Medical Center in Dubai explains that numbness from the anaesthetic lasts several hours, while nerve-related numbness, if it occurs, often improves gradually over weeks to months. Numbness beyond about 24 hours should be reported for follow-up.
When is coronectomy better than full wisdom tooth removal?
At Manhal Medical Center in Dubai, Dr. Moataz Al-Khan may consider coronectomy when CBCT shows the roots in close contact with the nerve and the tooth has no root infection. He explains the benefits and the small chance of needing a second procedure.
Does a CBCT scan really reduce the risk of nerve damage during wisdom tooth surgery?
Dr. Moataz Al-Khan at Manhal Medical Center in Dubai uses CBCT to see the nerve and roots in 3D, which helps plan the safest approach and decide whether coronectomy is appropriate. It improves planning, though no technique removes risk entirely.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staffAmazing service and very friendly team I'm very happy with my dental implant results with Dr.Manhal ❤️
