Alopecia areata injections — steroid injections and JAK-inhibitor options for patchy hair loss
Alopecia areata often appears suddenly as one or more smooth, round bald patches on the scalp, beard or eyebrows. It can be distressing, especially when patches spread, and people want to know which treatments are effective and safe.

Intralesional steroid injections are a common first-line treatment for limited patches of alopecia areata, given every four to six weeks to calm the immune attack on hair follicles; extensive or rapidly spreading cases may be considered for JAK-inhibitor tablets. At Manhal Medical Center in Dubai, Specialist Dermatologist Dr. Abdullah Anis diagnoses alopecia areata and plans treatment based on its extent.
Key facts
- Autoimmune hair loss in round patches; follicles remain alive
- Steroid injections every 4–6 weeks for limited patches
- Early regrowth often within 1–3 months
- JAK-inhibitor tablets for extensive cases, with monitoring
- Rapid spread or eyebrow loss needs prompt review
What alopecia areata is
Alopecia areata is an autoimmune condition in which the immune system targets hair follicles, causing hair to fall out in round or oval patches. The follicles are not destroyed, which is why regrowth is possible. It can affect the scalp, beard, eyebrows, eyelashes and body hair. Some people have a single patch that regrows on its own, while others have multiple patches, total scalp hair loss or loss of all body hair. It can run in families and is more common in people with other autoimmune conditions, such as thyroid disease or vitiligo, and in those with eczema. Nail pitting may also be seen.
How steroid injections work
Intralesional corticosteroid injections deliver a small amount of steroid directly into the skin of the bald patch, calming the local immune reaction so hair can regrow. They are typically used for limited patches in adults and older children. Very fine needles are used to place multiple small injections across the patch. Treatment is usually repeated every four to six weeks, and early regrowth is often seen within one to three months; the first new hairs may be fine or white before regaining their normal colour. Dr. Abdullah Anis checks the diagnosis, often with trichoscopy, before starting and reviews response at each session.
Side effects and safety of injections
Common side effects include brief discomfort during injections and small, temporary skin dents called atrophy at injection sites, which usually recover over months. Correct dilution, depth and spacing reduce this risk. Lightening of skin colour at injection sites can occur, particularly on darker skin. Injections are used more cautiously around the eyebrows and avoided in certain areas. If there is no response after several sessions, the treatment plan is reviewed rather than continuing indefinitely. Topical steroids, minoxidil or other topical treatments may be combined with injections to support regrowth. Each session is recorded so progress can be compared.
JAK inhibitors and other options for extensive disease
For extensive or rapidly progressive alopecia areata, including total scalp loss, oral JAK inhibitors have become an important option. These medicines block signalling pathways involved in the immune attack, and some, such as baricitinib and ritlecitinib, have been approved for severe alopecia areata in many countries. They require screening blood tests before starting and regular monitoring, because they can affect blood counts, cholesterol and infection risk. Other options include topical immunotherapy and, in selected cases, short courses of oral steroids. Dr. Abdullah Anis discusses benefits, risks, monitoring and availability so you can make an informed decision.
Course, support and when to seek review
Alopecia areata is unpredictable: patches may regrow and new ones appear, and treatment controls activity rather than offering a permanent cure. Associated conditions, especially thyroid disease, may be checked with blood tests. Emotional support matters, and wigs, hairpieces, brow techniques and camouflage products can help during treatment. Seek prompt review if hair loss spreads rapidly, if you lose eyebrows or eyelashes, if the scalp becomes red, scaly, painful or develops pustules, which may suggest a different diagnosis, or if you develop signs of infection while taking immune-modulating tablets. Regular follow-up helps adjust treatment as the condition changes.
Hair loss can come from genetics, hormones, iron or thyroid issues, stress, scalp inflammation or autoimmune alopecia. A dermatologist identifies the type before recommending treatment.

Reasonable price and great location. Great experience so far I had my laser done and very happy with the result. So I decided to purchase another package :)Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staff
