Is miswak good for teeth? Benefits for plaque, gums and whitening — and how to use it

Miswak has been used for centuries across the Arab and Muslim world and remains popular in the UAE, especially during Ramadan. People often ask whether miswak removes plaque, whether it whitens teeth, whether it contains fluoride and how it compares with miswak toothpaste.

Is miswak good for teeth? Benefits for plaque, gums and whitening — Manhal Medical Center, Dubai
Quick answer

Miswak, a chewing stick from the Salvadora persica (arak) tree, can help remove plaque and freshen the mouth, and studies suggest it has antibacterial properties and contains small amounts of natural fluoride. At Manhal Medical Center in Dubai, Dr. Fariba Alnoory recommends using miswak alongside, not instead of, brushing with fluoride toothpaste and cleaning between the teeth.

Key facts

  • Miswak can remove plaque from surfaces it reaches
  • Contains antibacterial compounds and small amounts of natural fluoride
  • Helps lift surface stains but does not bleach teeth
  • Use alongside, not instead of, fluoride toothpaste
  • Trim the tip regularly and replace dry or frayed sticks

What miswak is and how it works

Miswak, also called siwak, is a twig usually cut from the roots or branches of the arak tree. When the end is chewed, its fibres separate into a soft brush-like tip that can mechanically scrape plaque from tooth surfaces. Its natural compounds, which include traces of fluoride, silica, tannins and aromatic oils, have been studied for antibacterial and refreshing effects. The World Health Organization has acknowledged the chewing stick as an oral hygiene tool. Because it is portable and needs no water or toothpaste, many people use it during the day, after meals or while fasting.

Does miswak remove plaque and help gums?

Research comparing miswak with toothbrushes generally shows that miswak used correctly can remove plaque effectively from the surfaces it reaches, and some studies report reduced gum bleeding and bacterial counts. Its antibacterial compounds may contribute to healthier gums. However, a single stick cannot easily reach between teeth, behind the back molars or deep along the gum line, which is where gum disease often starts. Results also depend heavily on technique and how often the miswak is used. That is why it works well as a supplement to regular brushing and interdental cleaning, rather than a replacement.

Does miswak whiten teeth or contain fluoride?

Miswak can help remove some surface stains from tea, coffee and food, because of its gentle abrasive action and silica content, so teeth may look brighter and cleaner. It does not, however, change the natural colour of the tooth in the way professional whitening does. Miswak contains naturally occurring fluoride, but the amount released is small and variable compared with a standard fluoride toothpaste. For protection against decay, especially for children and people at higher risk, fluoride toothpaste remains important. Miswak-flavoured toothpastes combine arak extract with fluoride and can be a convenient option for those who like the taste.

How to use miswak correctly

Choose a fresh, moist stick and store it in a clean, dry place, ideally in a cover. Peel back about one centimetre of bark and chew the end gently until the fibres soften into bristles. Brush with short strokes across the teeth, holding the miswak at an angle along the gum line, and include the inner surfaces. Rinse the tip after use. Every day or two, cut off the used bristles and expose a fresh section so the tip stays clean and soft. Replace the stick when it becomes dry, frayed or short. Avoid pressing too hard, which can irritate the gums or wear exposed roots.

Combining miswak with dental care

A sensible routine for miswak users is to brush twice daily with a fluoride toothpaste, clean between the teeth once a day, and use miswak during the day, after meals or while fasting when brushing is less convenient. Regular check-ups and professional cleaning remain essential, because tartar cannot be removed with miswak or a toothbrush. At Manhal Medical Center, Dr. Fariba Alnoory can check your technique, look for areas the miswak is missing and advise on gum health. Contact the clinic if you notice bleeding gums, receding gums, sensitive teeth or persistent bad breath despite using miswak.

Professional cleaning removes hardened tartar that brushing can't, polishes away surface stains, and lets the dentist spot gum disease early.

Is miswak better than a toothbrush for cleaning teeth?

Dr. Fariba Alnoory, general dentist at Manhal Medical Center in Dubai, advises using miswak as a supplement rather than a substitute, because a toothbrush with fluoride toothpaste reaches more surfaces and provides stronger decay protection. Miswak is useful during the day and while fasting.

Can using miswak whiten my teeth naturally?

According to Dr. Fariba Alnoory at Manhal Medical Center in Dubai, miswak can lift some surface stains from tea or coffee so teeth look cleaner, but it does not change the natural tooth colour. Professional whitening is needed for a noticeable shade change.

Does miswak have enough fluoride to prevent cavities?

Dr. Fariba Alnoory at Manhal Medical Center in Dubai explains that miswak contains small, variable amounts of natural fluoride, so it should not replace fluoride toothpaste for decay prevention. Miswak toothpastes that include fluoride are an option.

What is the correct way to use a miswak stick?

Dr. Fariba Alnoory at Manhal Medical Center in Dubai suggests peeling about a centimetre of bark, chewing the end into soft bristles, brushing gently along the gum line and trimming the tip every day or two. She can check your technique at a routine visit.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
