Swollen lip, tongue or face after dental work — what is normal and when to worry
Waking up after dental treatment with a puffy lip or a lump under the tongue is worrying. Most of the time it is the body’s normal reaction to treatment, but it helps to know the difference between expected healing, a lip bite while numb, an allergic reaction and an infection.

Mild swelling of the lip, cheek or under the tongue after a filling, injection or extraction is common and usually settles within two to three days. Swelling that keeps growing after day three, comes with fever, pus or difficulty swallowing, or appears suddenly with hives or breathing trouble needs prompt review — Manhal Medical Center in Dubai can check it the same day.
Key facts
- Mild swelling after treatment often peaks at 48–72 hours
- Lip biting while numb is a common cause
- Swelling after day three may mean infection
- Sudden swelling with hives or breathing trouble is an emergency
- Cold compress on day one; warm salt rinses from day two
Why is my lip swollen after dental work?
The most common reasons are simple. Local anaesthetic injections can cause minor bruising or tissue reaction at the injection site. Stretching the lips during a long appointment can leave them puffy for a day. Many patients, especially children, accidentally bite or chew the lip while it is numb, which causes a swollen, sore and sometimes white ulcerated area a few hours later. After extractions or surgery, swelling is part of normal healing and usually peaks around 48 to 72 hours. Less commonly, swelling reflects an allergic reaction or an infection. The timing, the pattern and any other symptoms help your dentist tell which one it is.
Swelling under the tongue after dental work
Swelling under the tongue is usually related to lower-jaw treatment. A lower injection can cause bruising in the floor of the mouth, and work on lower molars or wisdom teeth can irritate the tissues there. Cotton rolls or suction can also bruise the delicate floor of the mouth. Occasionally a small salivary gland or duct is irritated and a soft swelling appears that comes and goes with meals. Mild, stable swelling that improves day by day is usually not serious. Swelling under the tongue that rises, makes the tongue lift, causes difficulty swallowing or speaking, or comes with fever is a warning sign of a spreading infection and needs urgent assessment, often in hospital.
Normal healing versus infection
Normal post-treatment swelling is greatest on the second or third day and then steadily reduces. It may be slightly tender and bruised but should not throb more and more. Infection usually behaves differently: swelling that keeps increasing after day three, or that appears several days after treatment, throbbing pain, a bad taste or pus, warmth and redness of the skin, swollen glands under the jaw and a raised temperature. An infection after an extraction or root canal treatment may need drainage, cleaning of the area and sometimes antibiotics prescribed by the dentist. Do not start leftover antibiotics on your own; the right treatment depends on the cause.
Allergic reactions: rare but important
True allergy to modern dental local anaesthetics is uncommon, but reactions can occur to latex gloves, some medicines, mouthwashes or materials. Allergic swelling tends to appear quickly, within minutes to hours, and can affect both lips, the eyelids or the whole face, often with itching, hives or a rash. Severe allergy can cause throat tightness, wheezing, dizziness or difficulty breathing. This is an emergency: call 998 or go to the nearest hospital emergency department immediately. Milder allergic swelling still needs to be reported so your dentist can record it and avoid the trigger in future treatment. Always tell the clinic about past reactions before any procedure.
What to do at home and when to call the clinic
For expected swelling, apply a cold compress wrapped in a cloth to the cheek for 10 to 15 minutes at a time during the first day, sleep with your head slightly raised, take the pain relief your dentist advised and avoid hot drinks and strenuous exercise for 24 hours. From the second day, gentle warm salt-water rinses can help after extractions unless told otherwise. If you bit your lip while numb, keep it clean and avoid spicy food. Contact Manhal Medical Center if swelling worsens after day three, if you see pus, have fever, or feel unsure. Seek hospital care straight away for breathing or swallowing difficulty.
Dental emergencies need quick relief and a clear diagnosis. We prioritise severe pain, swelling, trauma and broken restorations, and will tell you what to do while you travel to the clinic.

How long does a swollen lip last after dental work?
Mild swelling usually improves within two to three days; a bitten lip ulcer can take about a week to heal.
Should I use ice or heat?
Use a cold compress during the first 24 hours; avoid applying heat to a swelling that might be infected unless your dentist advises it.
Is it normal for my face to be bruised after an extraction?
Some bruising of the cheek or jaw can happen, especially after surgical extraction, and it usually fades over a week or so.
Can a filling cause swelling?
A filling itself rarely causes swelling; swelling after a filling is more often from the injection, a lip bite or an underlying nerve problem that needs checking.
Do I need antibiotics for swelling after dental work?
Only if the dentist diagnoses an infection; normal post-treatment swelling does not need antibiotics.

Why is my lip still swollen two days after a filling and is that normal?
Swelling for up to two or three days is often normal, says the dental team of Dr. Fariba Alnoory and Dr. Manhal Helfawee at Manhal Medical Center in Dubai, and is usually caused by the injection, stretching or biting the lip while numb. It should be getting better each day. If it is growing, very painful, or you have fever, have it checked.
I have swelling under my tongue after a lower tooth extraction — when should I worry?
Swelling under the tongue that is mild and slowly improving is usually part of healing, but Dr. Manhal Helfawee and Dr. Fariba Alnoory at Manhal Medical Center in Dubai advise urgent review if it keeps rising, lifts the tongue, or makes swallowing, speaking or breathing difficult. Those signs can indicate a spreading infection and may need hospital care.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staffAmazing service and very friendly team I'm very happy with my dental implant results with Dr.Manhal ❤️
