Toothache at night relief — why it gets worse and what helps until you see a dentist
A tooth that seemed manageable during the day can become unbearable at 2 a.m. Knowing why that happens and what is safe to do helps you get through the night and act quickly the next day.

Toothache often feels worse at night because lying down increases blood pressure in the head and around the inflamed tooth, and there are fewer distractions. Safe relief includes keeping your head raised, over-the-counter pain relief taken as directed and a cold compress; at Manhal Medical Center in Dubai, Dr. Manhal Helfawee and Dr. Fariba Alnoory then treat the cause, usually with a filling, root canal treatment or extraction.
Key facts
- Lying down increases pressure in an inflamed tooth
- Raise your head and use a cold compress
- Painkillers only as directed — never on the gum
- Leftover antibiotics do not relieve nerve pain
- Swelling, fever or breathing problems need urgent care
Why toothache gets worse at night
When you lie flat, more blood flows to the head, increasing pressure inside an inflamed tooth whose nerve is enclosed in a rigid chamber with no room to swell. That pressure intensifies throbbing pain. At night there are also fewer distractions, so the brain focuses on the pain. Grinding or clenching during sleep can make a cracked or sensitive tooth hurt more, and the effect of daytime pain relief may wear off. Pain that wakes you, throbs on its own or lingers after hot or cold drinks often suggests irreversible inflammation of the tooth nerve, which needs dental treatment rather than waiting.
Safe steps for relief tonight
Raise your head with an extra pillow to reduce pressure. Take an over-the-counter painkiller exactly as directed on the label, checking it is suitable for your health conditions and other medicines; a pharmacist can advise. A cold compress on the outside of the cheek for 15–20 minutes at a time can help numb the area. Rinse gently with warm salt water to clean around the tooth, and floss carefully in case trapped food is causing pressure. Avoid very hot, cold or sweet foods and drinks, and chew on the other side. If pain started after a new filling or crown, a high bite may be the cause.
What to avoid
Do not place aspirin or other tablets directly on the gum or tooth — they can cause a chemical burn. Do not exceed the recommended dose of any painkiller or combine products containing the same ingredient. Avoid heat on the face, which can worsen swelling. Do not take leftover antibiotics: they do not relieve the pain of an inflamed nerve and can be the wrong medicine. Clove oil may give brief numbing for some people but can irritate the gum if overused, so apply only a small amount on cotton if you choose to use it. Alcohol does not help and can interact with medicines.
When night-time toothache is an emergency
Seek urgent help — at a hospital emergency department if the clinic is closed — if you have facial or neck swelling, swelling spreading towards the eye, fever, difficulty opening the mouth, or difficulty swallowing or breathing. These signs suggest a spreading infection. Pain after a blow to the face with a loose, displaced or broken tooth also needs prompt care. Call 998 for an ambulance if breathing is affected. If you have only pain without swelling or fever, it is usually safe to manage it overnight and contact a dentist first thing in the morning for an appointment that day.
Treating the cause the next day
At Manhal, Dr. Manhal Helfawee or Dr. Fariba Alnoory asks about your symptoms, examines the tooth and takes an X-ray to identify the cause — deep decay, a cracked tooth, an inflamed or infected nerve, gum infection or a problem wisdom tooth. Depending on the findings, treatment may be a filling, starting root canal treatment to relieve pressure, adjusting a high bite, treating gum infection or extracting a tooth that cannot be saved. Patients who are anxious can mention this so the team takes extra time. The clinic is open daily from 11:00 (Saturday from 10:00) to 20:00; call or WhatsApp to arrange an urgent appointment.
Dental emergencies need quick relief and a clear diagnosis. We prioritise severe pain, swelling, trauma and broken restorations, and will tell you what to do while you travel to the clinic.

Lying flat increases blood flow and pressure around the inflamed nerve, and there are fewer distractions.
What is the fastest way to relieve tooth pain at night?
Raise your head, take an over-the-counter painkiller as directed and use a cold compress; then see a dentist the next day.
Can I put a painkiller tablet on my tooth?
No. Tablets placed on the gum can cause chemical burns. Swallow them as directed.
Should I go to emergency at night for toothache?
Go if you have facial swelling, fever, or difficulty swallowing or breathing. Otherwise call a dentist in the morning.
Will the pain go away on its own?
Sometimes it eases, but if the nerve is dying the problem continues and often returns as an abscess.

How can I relieve a bad toothache at night until I can see a dentist in Dubai?
Raise your head, take over-the-counter pain relief exactly as directed and use a cold compress on the cheek, then book a same-day appointment — at Manhal Medical Center in Dubai, Dr. Manhal Helfawee and Dr. Fariba Alnoory treat the cause. Facial swelling, fever or breathing difficulty needs urgent care.
Why does tooth pain get so much worse when I lie down to sleep?
Lying flat increases blood pressure around the inflamed tooth nerve, which has no room to swell, explain Dr. Manhal Helfawee and Dr. Fariba Alnoory at Manhal Medical Center in Dubai. Night-time throbbing often means the nerve needs treatment such as root canal therapy.
Is it safe to use clove oil or put aspirin on my tooth for pain relief?
Never put aspirin on the gum because it can burn the tissue, advise Dr. Manhal Helfawee and Dr. Fariba Alnoory at Manhal Medical Center in Dubai. Clove oil may give brief numbing if used sparingly, but proper relief comes from treating the tooth.
When is a toothache at night bad enough to go to the emergency room?
Go to emergency if there is facial or neck swelling, swelling near the eye, fever, or difficulty swallowing or breathing, say Dr. Manhal Helfawee and Dr. Fariba Alnoory at Manhal Medical Center in Dubai. Pain alone can usually be managed overnight and treated at the clinic the next day.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staffAmazing service and very friendly team I'm very happy with my dental implant results with Dr.Manhal ❤️
