Face swelling from a tooth — danger signs, how long it lasts and urgent treatment
Waking up with a swollen cheek or jaw is alarming, and people often ask whether it is dangerous and how long it will last. The answer depends on the cause and how far the infection has spread, which is why swelling should never simply be waited out.

Facial or cheek swelling from a tooth usually means a dental abscess that needs prompt dental treatment, and it can become dangerous if it spreads. At Manhal Medical Center in Dubai, Dr. Manhal Helfawee and Dr. Fariba Alnoory see swelling cases urgently during opening hours; swelling with difficulty breathing or swallowing, or spreading to the eye or neck, needs a hospital emergency department immediately.
Key facts
- Facial swelling from a tooth usually means an abscess
- Breathing or swallowing difficulty needs emergency care
- Swelling reaching the eye or neck is a red flag
- Cold compress, not heat; never burst it yourself
- Treatment drains the infection and removes the cause
Why teeth cause facial swelling
Most facial swelling from teeth is caused by infection. Bacteria from a dead or deeply decayed tooth, a failed root canal, a deep gum pocket or a partially erupted wisdom tooth form an abscess, and pus spreads into the soft tissues of the cheek, jaw or under the chin. Swelling can also follow recent dental surgery, where some swelling is normal and peaks around day two or three, or be caused by trauma. Less commonly, blocked salivary glands or other conditions cause swelling near the jaw. A dentist distinguishes these with an examination and X-rays, because the treatment for each is different.
Is swelling from a tooth dangerous?
It can be. Most dental abscesses are managed safely with prompt treatment, but infection can spread to deeper spaces of the face and neck. Danger signs include swelling that is spreading quickly, swelling under the jaw or tongue, swelling reaching the eye so that it closes, difficulty opening the mouth, difficulty swallowing or breathing, a raised voice-changing tongue, high fever, and feeling very unwell. These can indicate a serious spreading infection that needs hospital care. People with diabetes, weakened immunity or other health conditions should seek help especially early. Do not wait for antibiotics from a previous illness to ‘see if it settles’.
How long does cheek swelling last?
Swelling from a dental abscess does not usually go away permanently on its own; it may fluctuate or drain into the mouth, but the infection remains until the source is treated. After drainage and treatment of the tooth, swelling typically begins to improve within one to three days and settles over about a week, depending on its size. If antibiotics were also needed, improvement is usually noticeable within a couple of days. Swelling after planned surgery, such as an extraction or implant, usually peaks at 48–72 hours and then fades. Swelling that keeps increasing beyond that, or returns, should be reviewed promptly.
What to do now
If you have any danger signs — difficulty breathing or swallowing, swelling spreading to the eye or neck, or high fever with feeling very unwell — go to the nearest hospital emergency department or call 998 for an ambulance. Otherwise, contact a dentist the same day. Meanwhile, keep your head raised, use a cold compress on the outside of the face for short periods, take over-the-counter pain relief as directed on the label, keep drinking fluids and rinse gently with warm salt water. Do not apply heat to the outside of the face, and never try to burst a swelling yourself. Avoid driving if you feel unwell.
How Manhal treats dental swelling
At Manhal, Dr. Manhal Helfawee or Dr. Fariba Alnoory examines the swelling, checks your temperature, mouth opening and swallowing, and takes X-rays to find the source. Treatment aims to remove the cause and drain the infection: this may involve opening the tooth to start root canal treatment, draining an abscess through the gum, or extracting a tooth that cannot be saved. Antibiotics are prescribed when the infection is spreading or you have systemic signs. Dr. Moataz Al-Khan, Specialist Oral & Maxillofacial Surgeon, may be involved for surgical cases. If hospital care is needed, you will be referred straight away. The clinic is open daily from 11:00 (Saturday from 10:00) to 20:00.
Dental emergencies need quick relief and a clear diagnosis. We prioritise severe pain, swelling, trauma and broken restorations, and will tell you what to do while you travel to the clinic.

How long does cheek swelling from a tooth last?
It usually improves within one to three days after the infection is drained and the tooth treated, and settles over about a week.
Should I use a hot or cold compress?

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staffAmazing service and very friendly team I'm very happy with my dental implant results with Dr.Manhal ❤️
