Alternatives to root canal — pulpotomy, extraction, implants and bridges

Patients search “بديل حشو العصب” when they are worried about pain, cost or time. It is reasonable to ask about alternatives; the key is understanding the long-term trade-offs of each.

Alternatives to root canal — Manhal Medical Center, Dubai
Quick answer

The main alternatives to a root canal are vital pulp therapy such as pulpotomy in selected cases where the nerve is still healthy enough, or extraction followed by replacing the tooth with an implant, bridge or denture, or leaving the gap. Each option has consequences for function, neighbouring teeth, bone and cost over time. At Manhal Medical Center in Dubai, Dr. Manhal Helfawee explains which alternatives are realistic for your tooth and what each would involve.

Key facts

  • Vital pulp therapy may save a still-healthy nerve
  • Pulpotomy suits selected cases, not dead or infected nerves
  • Extraction removes infection but leaves a gap
  • Implants, bridges and dentures can replace a tooth
  • Leaving a gap can cause tipping and bone loss

When a root canal is recommended

Root canal treatment is advised when the nerve inside a tooth is irreversibly inflamed or has died and become infected, often because of deep decay, a crack, trauma or repeated fillings. Signs can include lingering pain to hot or cold, spontaneous or night-time pain, pain on biting, swelling or a gum boil, though some infected teeth have no symptoms and are found on X-rays. The aim is to remove the source of infection while keeping the natural tooth. Before considering alternatives, it helps to confirm the diagnosis with tests and imaging, because the right options depend on whether the nerve can still recover.

Vital pulp therapy and pulpotomy

If the nerve is inflamed but still healthy enough to heal, a more conservative approach may be possible. Vital pulp therapy aims to keep part or all of the nerve alive. Options include indirect or direct pulp capping, where a medicated material is placed over or on a small exposure of the nerve, and pulpotomy, where the inflamed upper part of the nerve is removed and the remaining healthy tissue is covered with a biocompatible material. Pulpotomy is very common in children's teeth and increasingly considered in selected adult cases. Success depends on correct case selection, so it is not suitable for teeth with long-standing infection or dead nerves.

Extraction and replacing the tooth

Removing the tooth eliminates the infection, and sometimes extraction is the most sensible choice, for example when a tooth cannot be restored or has a vertical root fracture. After extraction, the gap can be replaced with a dental implant, a fixed bridge or a removable denture. An implant replaces the root and does not involve cutting neighbouring teeth, but it requires surgery, healing time and enough bone. A bridge uses the adjacent teeth as supports, which means preparing them for crowns. A denture is the least invasive option but is removable and less stable. Each involves more time and cost than saving a restorable tooth.

Leaving the gap: what happens over time

Some patients choose extraction without replacement, particularly for back teeth that are not visible. This is an option, but it has consequences. Neighbouring teeth can gradually tip or drift into the space, the opposing tooth may over-erupt, food can trap more easily, and chewing may shift to the other side. The bone where the tooth was usually shrinks over time, which can make future implant placement more complex and may require grafting. These changes happen slowly and vary between people. Discussing them before extraction helps you decide whether leaving the gap is acceptable or whether replacement should be planned.

Choosing the right option at Manhal

The right choice depends on the tooth's condition, how restorable it is, your general health, your priorities and long-term plans. At Manhal Medical Center in Dubai, Dr. Manhal Helfawee, a Consultant Implantologist, examines the tooth with X-rays and, where useful, 3D CBCT imaging, and explains root canal treatment alongside realistic alternatives, including vital pulp therapy where suitable, extraction and implant, bridge or denture options. Each option is described in a written, itemised plan with its benefits and trade-offs. Seek urgent care if you have facial swelling, fever or difficulty swallowing, as infection should not be left untreated while you decide.

Root canal treatment removes infected or inflamed pulp from inside the tooth, cleans and shapes the canals, and seals them. It relieves toothache and allows you to keep your natural tooth instead of extracting it.

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.

Is there an alternative to root canal?

Yes, depending on the case: vital pulp therapy, or extraction with or without replacing the tooth.

Can a pulpotomy replace a root canal in adults?

In selected adult cases with a still-healthy nerve, a pulpotomy may be considered. It is not suitable for infected or dead nerves.

Is extraction cheaper than a root canal?

Extraction alone may be, but replacing the tooth with an implant or bridge usually costs more over time.

What happens if I do nothing about an infected tooth?

Infection can spread, cause abscesses and pain, and in rare cases become serious. It should be treated.

Is an implant better than a root canal?

Saving a restorable natural tooth is usually preferred; implants are an excellent option when a tooth cannot be saved.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
