Pityriasis rosea — herald patch, Christmas-tree rash, course and relief

Pityriasis rosea often alarms people because a single scaly patch is followed days later by dozens of new spots across the chest and back. It is most common in teenagers and young adults and is frequently mistaken for ringworm or eczema.

Pityriasis rosea — Manhal Medical Center, Dubai
Quick answer

Pityriasis rosea is a common, harmless rash that usually starts with one larger oval herald patch followed by smaller patches on the trunk in a Christmas-tree pattern, and it typically clears on its own within six to twelve weeks. At Manhal Medical Center in Dubai, Dr. Abdullah Anis confirms the diagnosis, rules out look-alike rashes and eases the itch.

Key facts

  • Starts with one larger oval herald patch
  • Smaller patches follow on the trunk in a Christmas-tree pattern
  • Usually clears on its own in 6–12 weeks
  • Ringworm, psoriasis and syphilis can look similar
  • Pregnant patients should inform their obstetrician

The herald patch and Christmas-tree rash

The rash usually begins with a single oval, pink or salmon-coloured patch, often two to five centimetres across, on the chest, back or abdomen. This is called the herald patch and it frequently has a fine collar of scale around its edge. One to two weeks later, many smaller oval patches appear on the trunk and upper arms. On the back they tend to line up along the skin creases, creating the pattern often described as a Christmas tree. On darker skin the patches may look grey, brown or more raised. Some people feel mildly unwell, with tiredness or a sore throat, before the rash appears.

What causes pityriasis rosea

The exact cause is not fully understood, but evidence suggests a link with reactivation of human herpesviruses 6 and 7, which most people carry from childhood. It is not considered highly contagious, and it is not caused by fungus, bacteria, food or poor hygiene, so isolation from school or work is not usually needed. Certain medicines can occasionally cause a similar rash, so Dr. Abdullah Anis will ask about any new medication. Most people only have pityriasis rosea once, although recurrence is possible. Knowing the likely cause helps many patients feel reassured while they wait for the rash to fade.

Look-alike rashes that need ruling out

Several conditions can resemble pityriasis rosea, and distinguishing them matters because their treatment is different. Ringworm (tinea) can look like a herald patch but needs antifungal treatment. Guttate psoriasis, nummular eczema and drug eruptions can produce many small patches. Secondary syphilis can also cause a widespread rash, particularly involving the palms and soles, so a blood test may be recommended in some cases, handled confidentially. Dr. Abdullah Anis examines the pattern and scale carefully and may take a skin scraping or arrange tests if the picture is not typical. This avoids using the wrong cream.

How long it lasts and how to relieve it

Pityriasis rosea usually settles without treatment over six to twelve weeks, occasionally longer. Treatment focuses on comfort. Gentle soap-free washing, lukewarm rather than hot showers, and fragrance-free moisturisers help reduce irritation. Itch can be eased with antihistamines and, if needed, a mild to moderate steroid cream prescribed for a short period. In more extensive or very itchy cases, the dermatologist may discuss additional options, including referral for phototherapy. Heat, sweating and vigorous exercise can make the rash look redder temporarily. Marks that remain on darker skin generally fade over the following months.

Pregnancy and when to seek review

If you are pregnant and develop a rash that looks like pityriasis rosea, tell your obstetrician promptly, because some studies have suggested a link with pregnancy complications when it occurs in early pregnancy. You should also return for review if the rash lasts longer than about three months, involves the palms, soles or mouth, becomes blistered or very painful, or if you feel significantly unwell with fever. These features suggest a different diagnosis may need to be considered. Otherwise, reassurance and simple itch relief are often all that is required.

Eczema and psoriasis are chronic inflammatory skin conditions. Good control comes from an accurate diagnosis, a clear plan for flares, and daily skin care that suits the UAE climate.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
