Permanent filler removal in Dubai — when hyaluronidase cannot dissolve it

Some people had fillers years ago, sometimes abroad, without knowing what product was used. Later they notice hard lumps, swelling that comes and goes, or a shape that no longer suits their face, and discover that the filler does not respond to dissolving injections.

Permanent filler removal in Dubai — Manhal Medical Center, Dubai
Quick answer

Permanent and semi-permanent fillers cannot be dissolved with hyaluronidase, so removal depends on identifying the product, often with soft-tissue ultrasound, and then treating lumps or inflammation with injections and medicines or, when needed, surgical removal by an appropriate specialist. At Manhal Medical Center in Dubai, Dr. Abdullah Anis assesses non-dissolvable filler problems and plans the safest route.

Key facts

  • Permanent fillers cannot be dissolved by hyaluronidase
  • Identifying the product is the first step
  • Ultrasound imaging helps locate filler and guide decisions
  • Nodules may respond to injections and medicines
  • Surgical removal is considered selectively, with referral

Dissolvable vs non-dissolvable fillers

Most modern facial fillers are made of hyaluronic acid, which can be broken down with an enzyme called hyaluronidase. Other products behave differently. Semi-permanent biostimulatory fillers, such as calcium hydroxylapatite or poly-L-lactic acid, gradually stimulate collagen and cannot be dissolved by the enzyme. Permanent fillers, such as silicone, polymethyl methacrylate microspheres or polyacrylamide gels, are designed to stay in the tissue indefinitely. When a permanent filler causes a problem, it cannot simply be reversed, which is why searching for permanent filler removal in Dubai usually leads to a more detailed medical assessment rather than a single quick treatment.

Finding out what filler you have

The first step is to establish, as far as possible, what was injected, where and when. Records from the original clinic, product stickers or messages can be very helpful. Dr. Abdullah Anis examines the face, feels the texture and depth of lumps and asks about episodes of swelling, redness or pain. Soft-tissue ultrasound is often used in filler practice to show where the material sits and how it looks, which helps distinguish hyaluronic acid from other products; when imaging is needed it is arranged as part of the plan. A test injection of hyaluronidase may be considered if a hyaluronic acid component is suspected.

Treating lumps, nodules and granulomas

Permanent fillers can cause firm nodules, delayed inflammatory swelling or granulomas, which are areas of chronic inflammation around the foreign material. These may flare after infections, dental treatment or vaccinations. Depending on the type of reaction, treatment can include steroid injections into the nodule, anti-inflammatory or antibiotic courses when a low-grade infection or biofilm is suspected, and other medicines that calm chronic inflammation. Dr. Abdullah Anis, whose background includes a fellowship in dermatopathology, may suggest a biopsy in selected cases to clarify what is happening in the tissue before deciding on longer treatment.

When surgical removal is considered

When a permanent filler forms a well-defined collection, keeps causing inflammation despite medical treatment, or distorts facial appearance, surgical removal may be discussed. This depends on the location, how the product has spread through the tissue and the risk of damaging nerves, vessels or the skin. Complete removal is not always possible, because some permanent products infiltrate surrounding tissue. In such cases, Dr. Abdullah Anis explains the realistic aims, coordinates imaging and refers to an appropriate surgical specialist, while continuing to manage inflammation medically before and after any procedure. Decisions are made jointly, step by step.

Red flags and choosing a safe path

Seek urgent care if a filled area suddenly becomes hot, red, very painful or swollen, especially with fever, as this may signal infection. Any change in skin colour, blanching or severe pain soon after a filler injection is an emergency that needs immediate medical attention. For non-urgent concerns, avoid further filler in the same area until the existing product is understood, because mixing materials can complicate diagnosis. Bringing photos taken before and after the original treatment, plus any records, helps the dermatologist assess your case and build a written plan.

Filler complications range from lumps and delayed swelling to biofilm infection, granulomas and, rarely, vascular occlusion that threatens skin or vision. Our dermatologist assesses complications from treatments done anywhere and builds a management plan based on published treatment algorithms.

Questions patients ask

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.

Can permanent filler be dissolved?

No. Hyaluronidase only breaks down hyaluronic acid fillers. Permanent fillers need other medical or surgical approaches.

How do I know which filler I had?

Records or product stickers from the original clinic help most. When these are not available, examination and imaging can provide clues.

Why has my old filler started swelling?

Delayed inflammatory reactions can occur years later, sometimes triggered by infections, dental work or vaccinations. They need medical assessment.

Can all permanent filler be removed surgically?

Not always. Some products spread into surrounding tissue, so complete removal may not be possible without risk to nearby structures.

Should I have more filler on top to hide lumps?

It is usually better to avoid adding filler until the existing product and reaction are understood, as this can make diagnosis and treatment harder.

My filler was done years ago abroad and now has hard lumps — what should I do?

Bring any records you have to Dr. Abdullah Anis at Manhal Medical Center in Dubai, who examines the lumps and may arrange ultrasound imaging to locate the filler. Treatment then depends on whether the product is dissolvable and what type of reaction is present.

Does hyaluronidase work on silicone or PMMA filler?

No, hyaluronidase only dissolves hyaluronic acid filler, explains Dr. Abdullah Anis at Manhal Medical Center, Dubai. Silicone, PMMA and similar permanent products need different medical or surgical approaches.

What is a filler granuloma and how is it treated?

A granuloma is chronic inflammation around filler material, and Dr. Abdullah Anis at Manhal Medical Center in Dubai treats it with steroid injections, anti-inflammatory or antibiotic courses depending on the cause. A biopsy may be suggested in selected cases.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
