How soon after extraction can I get an implant? Immediate, early or delayed

If you are about to lose a tooth, or lost one long ago, timing is one of the first questions about implants. The right answer depends on infection, bone and the position of the tooth, not on a fixed rule.

How soon after extraction can I get an implant? Immediate, early or delayed — Manhal Medical Center, Dubai
Quick answer

An implant can sometimes be placed on the same day as the extraction (immediate placement), more often after about six to twelve weeks of gum healing (early placement), or after three to six months or more when infection or bone loss needs time to heal (delayed placement). Implants can also be placed years later, although bone grafting is more often needed. At Manhal Medical Center in Dubai, Dr. Manhal Helfawee and Dr. Moataz Al-Khan decide timing from the examination and CBCT scan.

Key facts

  • Immediate placement is possible when there is no infection and good bone
  • Early placement is usually around six to twelve weeks after extraction
  • Delayed placement: three to six months or more
  • Socket preservation grafting helps limit bone loss
  • Implants are often still possible years after extraction

Immediate implant placement (same day)

In immediate placement, the tooth is removed gently and the implant is placed into the socket at the same appointment. The main benefits are fewer surgical visits and a shorter overall timeline, and in the front of the mouth it may help support the gum shape. However, it is only suitable when there is no acute infection, the socket walls are intact, and there is enough bone beyond the root tip to anchor the implant firmly. Gaps around the implant may be filled with graft material. Sometimes a temporary tooth can be attached immediately, though it is kept out of heavy biting contact while healing.

Early placement (after soft-tissue healing)

Early placement means waiting around six to twelve weeks after extraction, allowing the gum to close over the socket and any minor infection to settle, while most of the surrounding bone is still preserved. This approach is commonly used when immediate placement is not ideal, for example after an infected tooth, a thin bone wall or a multi-rooted molar where the socket is large. It gives better soft tissue to work with and makes infection-free placement more predictable. Bone grafting at the time of implant placement may still be needed to rebuild the contour of the jaw.

Delayed placement and socket preservation

Delayed placement means waiting three to six months or more after extraction so the bone fully heals. It is chosen when there was significant infection, large bone defects, a cyst, or when medical or personal reasons make waiting sensible. To limit the natural shrinkage of bone after extraction, a socket preservation graft may be placed at the time of removal, filling the socket with bone substitute and covering it. This helps maintain bone width so a standard implant is more likely to be possible later. Dr. Moataz Al-Khan, Specialist Oral & Maxillofacial Surgeon, often performs difficult extractions and grafting in these cases.

Can you get an implant years after an extraction?

Yes, implants can often be placed many years after a tooth was removed. The challenge is that bone gradually shrinks in height and width after extraction, and in the upper back jaw the sinus may expand downwards. Neighbouring teeth may also have tipped into the gap and the opposing tooth may have over-erupted, reducing space. A CBCT scan shows how much bone remains. Options include bone grafting, a sinus lift, a short or narrow implant, or minor orthodontic movement to reopen space. Waiting years does not usually rule implants out, but it can add steps and time to treatment.

How timing is decided at Manhal Medical Center

Timing is decided by examining the tooth, gum and bite, reviewing your medical history and taking a CBCT 3D scan. Key questions are whether the tooth is infected, how thick the bone walls are, whether there is enough bone beyond the root, how visible the area is and your own priorities for speed and appearance. Dr. Manhal Helfawee, Consultant Implantologist, and Dr. Moataz Al-Khan then explain the realistic options and the overall timeline, from extraction to final crown, in a written plan. If you are planning travel or events, mention them at consultation so treatment stages can be scheduled sensibly.

A dental implant replaces the root of a missing tooth with a biocompatible titanium post that bonds with the jawbone. Once integrated, it supports a crown that looks, feels and functions like a natural tooth, and it helps prevent the bone loss that follows an extraction.

How long should I wait between extraction and implant?

Commonly six to twelve weeks for early placement, or three to six months or more if bone needs to heal.

Is it too late to get an implant years after losing a tooth?

Usually not, but bone grafting or a sinus lift may be needed if the bone has shrunk.

Does waiting cause bone loss?

Yes, bone typically shrinks after extraction, which is why socket preservation is sometimes recommended.

Can I get an implant on the same day my tooth is pulled in Dubai?

At Manhal Medical Center in Dubai, Dr. Manhal Helfawee and Dr. Moataz Al-Khan offer immediate implant placement when there is no acute infection, the socket walls are intact and the bone can stabilise the implant. If not, they recommend early or delayed placement, decided from the CBCT scan.

Is it still possible to get an implant ten years after my tooth was extracted?

Often yes. Dr. Manhal Helfawee at Manhal Medical Center, Dubai, assesses remaining bone on a CBCT scan; if it has shrunk, bone grafting, a sinus lift or a short or narrow implant may be planned. Waiting years mainly adds steps rather than ruling implants out.

What is socket preservation and do I need it after extraction?

According to Dr. Moataz Al-Khan at Manhal Medical Center in Dubai, socket preservation is a graft placed into the socket at extraction to limit bone shrinkage. It is useful when an implant is planned later but cannot be placed immediately.

How long is the total time from extraction to implant crown?

Dr. Manhal Helfawee at Manhal Medical Center in Dubai explains that it can range from about three to four months with immediate placement to nine months or more when socket healing and grafting are needed first. He gives a personalised timeline in your written plan.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
