Upper implant overdenture palate free — a denture without covering the roof of the mouth
Many upper denture wearers dislike the plastic covering their palate: it can dull taste and temperature, trigger gagging and feel bulky when speaking. Implants can provide the support that allows that palate section to be removed.

A palate-free upper implant overdenture is a removable upper denture that clips onto usually four implants, so it no longer needs to cover the roof of the mouth for suction. At Manhal Medical Center in Dubai, Consultant Implantologist Dr. Manhal Helfawee assesses bone and sinus anatomy with a CBCT scan to see whether this design suits you.
Key facts
- Usually four upper implants support the denture
- No palate coverage — taste and comfort can improve
- Locators or a milled bar hold the denture
- CBCT checks bone and sinus before planning
- Removable for daily cleaning
Why conventional upper dentures cover the palate
A conventional upper denture relies on a thin layer of saliva and a seal around its edges to create suction, and the large surface of the palate is what makes that suction effective. Removing the palate from a conventional denture usually makes it fall down. For many wearers the palate coverage is tolerable, but others find it affects the taste and temperature of food and drink, makes them gag, or makes speech feel thick. The palate also contains many sensory receptors, which is why its coverage is noticed so much. A palate-free design needs support and retention from somewhere else — which is where implants come in.
How a palate-free implant overdenture works
Typically four implants are placed in the upper jaw, spaced around the arch. Because upper jaw bone is often softer than lower jaw bone, more implants are usually needed than for a lower overdenture. The implants are then either fitted with individual attachments such as locators, or connected with a custom milled bar. The denture is made in a horseshoe shape with clips that engage the attachments or bar, so it is held by the implants rather than by palate suction. It remains removable for cleaning, giving a balance between security, comfort and hygiene.
Benefits for taste, comfort and speech
With the roof of the mouth uncovered, many patients report improved enjoyment of the taste and temperature of food, less gagging and a less bulky feeling. Speech may feel more natural once you adapt. The denture is also more stable during chewing because it is anchored to implants rather than relying on suction that can break when biting into firm foods. Some patients who could not tolerate a conventional upper denture find a palate-free design much more manageable. As with any new denture, there is an adjustment period of a few weeks while the tongue, cheeks and lips get used to the new shape.
Requirements: bone, sinus and space
The upper jaw has the maxillary sinus above the back teeth area, and bone there can be limited after tooth loss. A CBCT scan shows how much bone is available and where implants can safely be placed; sometimes implants are positioned towards the front of the jaw or angled, and sometimes a sinus lift or bone graft is needed. Enough space between the jaws is required for the attachments and the denture teeth, especially if a bar is used. Patients must be medically fit for minor oral surgery. Dr. Moataz Al-Khan, Specialist Oral & Maxillofacial Surgeon at Manhal, may be involved where grafting or a sinus lift is required.
Treatment stages and maintenance
Treatment starts with a consultation, CBCT imaging and a discussion of whether a removable overdenture or fixed implant bridge suits your priorities. Implants are placed under local anaesthesia and allowed to integrate over a few months, while you wear a temporary or adjusted denture. The new palate-free denture is then designed, tried in and fitted to the attachments or bar. Clean the denture and the implant attachments daily and remove the denture at night unless advised otherwise. Clips are replaced periodically and the implants reviewed every six to twelve months. Cost depends on the number of implants, grafting and attachment type, with a written plan after examination.
Modern dentures are designed for fit, bite and appearance. When a lower denture moves, two or more implants can hold it in place with click-in attachments.

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.
Why do upper overdentures need four implants?
Upper jaw bone is often softer and there is no palate suction, so four implants are usually needed to hold the denture securely.
Will food taste better without the palate?
Many patients report better taste and temperature sensation once the palate is uncovered, though experiences vary.
Can I convert my current upper denture to palate-free?
Sometimes, if it is in good condition and the implants are placed to suit it; often a new denture is made.
Do I need a sinus lift?
Only if the CBCT scan shows there is not enough bone in the planned implant positions.
Is a palate-free overdenture fixed?
No. It clips onto the implants but you remove it for cleaning.

Can I get an upper denture without the palate in Dubai?
Yes, with implant support — at Manhal Medical Center in Dubai, Consultant Implantologist Dr. Manhal Helfawee provides palate-free upper overdentures that usually clip onto four implants. A CBCT scan first checks bone and sinus anatomy to confirm suitability.
How many implants does a palate-free upper denture need?
Usually four, explains Dr. Manhal Helfawee at Manhal Medical Center in Dubai, because upper jaw bone is often softer and the denture can no longer rely on palate suction. The exact number and positions are planned from a CBCT scan.
Is a palate-free implant overdenture better than a fixed implant bridge?
Neither is better for everyone, says Dr. Manhal Helfawee at Manhal Medical Center in Dubai: the overdenture is removable for easier cleaning and can provide lip support, while a fixed bridge never comes out but needs more meticulous hygiene. He discusses both after examination.
I gag with my upper denture — will implants help?
Often they can, because a palate-free design supported by implants removes the plastic from the roof of the mouth, explains Dr. Manhal Helfawee at Manhal Medical Center in Dubai. Suitability depends on bone and sinus anatomy seen on a CBCT scan.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staffAmazing service and very friendly team I'm very happy with my dental implant results with Dr.Manhal ❤️
