Diaper rash treatment — irritant vs candida nappy rash and when to see a dermatologist
Nappy rash is one of the most common skin problems in babies and can make them uncomfortable and unsettled, especially during teething, diarrhoea or after antibiotics. In the UAE heat, nappies can become warm and humid quickly, and parents often want to know which cream to use and when to worry.

Most diaper rash is irritant rash caused by moisture and friction and improves within a few days with frequent nappy changes, nappy-free time and a barrier cream; a bright red rash with small spots at the edges may be candida (yeast) and needs antifungal treatment. At Manhal Medical Center in Dubai, specialist dermatologist Dr. Abdullah Anis assesses persistent or unusual nappy rashes in babies and toddlers.
Key facts
- Most nappy rash is irritant and settles in days
- Candida rash involves folds with satellite spots
- Frequent changes and barrier cream are key
- Avoid strong steroid creams unless prescribed
- Fever, blisters or pus need prompt review
Irritant diaper rash
Irritant diaper rash develops when the skin is in prolonged contact with urine and stool, which break down the skin barrier, especially when combined with friction from the nappy and heat. It usually appears as red, sometimes shiny patches on the parts of the skin that touch the nappy most, such as the buttocks, outer genitals and upper thighs, while the deep skin folds are often spared. It may flare during teething, diarrhoea or when a baby starts new foods. Irritant rash usually responds well to good nappy hygiene and a protective barrier cream within a few days.
Candida (yeast) nappy rash
Candida rash is caused by an overgrowth of yeast, which thrives in warm, moist areas. It often looks bright red and raw, tends to involve the skin folds, and has small red spots or pustules scattered around the edges, sometimes called satellite lesions. It is more likely if a rash lasts more than two or three days, after a course of antibiotics, or when the baby also has white patches in the mouth (oral thrush). Candida rash needs an antifungal cream, and barrier creams alone are usually not enough. Diagnosis by a doctor helps make sure the right treatment is used.
Home care that helps
Change nappies frequently, and as soon as possible after a bowel movement. Clean gently with lukewarm water and a soft cloth or fragrance-free, alcohol-free wipes, and pat the skin dry rather than rubbing. Allow some nappy-free time each day so the skin can breathe, which can be easier on a towel in an air-conditioned room. Apply a thick layer of a barrier cream or ointment, such as zinc oxide or petrolatum, at each change; there is no need to scrub it all off each time. Make sure nappies are not too tight, and avoid talcum powder and perfumed products. Do not use strong steroid creams unless prescribed.
Other causes of nappy-area rashes
Not all rashes in the nappy area are simple nappy rash. Seborrhoeic dermatitis can cause red, well-defined patches in the folds and often on the scalp as cradle cap. Atopic eczema, psoriasis and allergic reactions to wipes or nappy materials can also appear here. Bacterial infections, such as impetigo or streptococcal infection around the anus, cause crusting, blisters or bright red, sore areas and need specific treatment. Rarely, persistent rashes can be a sign of other medical conditions or nutritional problems. When a rash does not behave like typical nappy rash, Dr. Abdullah Anis can examine the skin and recommend tests or treatment.
When to see a doctor urgently
See a doctor if the rash does not improve after three days of good care, keeps coming back, spreads beyond the nappy area, or your baby seems very uncomfortable. Seek prompt or urgent medical attention if there are blisters, pus-filled spots, open sores, crusting or bleeding, if your baby has a fever, is unusually sleepy, irritable or feeding poorly, or if the rash appears after starting a new medicine. Newborns under three months with fever should always be seen urgently. Dr. Abdullah Anis, a specialist dermatologist with more than 25 years of experience, sees babies and children with persistent or unusual skin rashes.
Eczema and psoriasis are chronic inflammatory skin conditions. Good control comes from an accurate diagnosis, a clear plan for flares, and daily skin care that suits the UAE climate.

Reasonable price and great location. Great experience so far I had my laser done and very happy with the result. So I decided to purchase another package :)Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staff
