Baby bottle tooth decay — what it looks like, treatment and prevention

Parents often notice white or brown marks on their toddler's upper front teeth and search for what bottle decay looks like. Catching it at the white-spot stage gives a good chance of stopping it without fillings.

Baby bottle tooth decay — Manhal Medical Center, Dubai
Quick answer

Baby bottle tooth decay is early childhood tooth decay caused when milk, formula or juice stays on a baby's teeth for long periods, especially during night feeds. It usually starts as chalky white lines near the gums of the upper front teeth and can progress to brown or broken teeth. At Manhal Medical Center in Dubai, Dr. Fariba Alnoory checks young children's teeth, treats decay gently and helps parents adjust feeding and brushing routines.

Key facts

  • Usually starts as chalky white lines on upper front teeth
  • Linked to frequent sugary or milk feeds, especially at night
  • Early white spots can often be stopped with fluoride
  • Brush with a rice-grain smear of fluoride toothpaste from the first tooth
  • Facial swelling or fever with tooth pain needs urgent care

What baby bottle tooth decay looks like

The upper front teeth are usually affected first, because the tongue covers the lower front teeth during feeding. The earliest sign is a dull, chalky white band along the gum line, which many parents mistake for normal colour. Over weeks to months these areas can turn yellow or brown, become rough and start to break away, sometimes leaving short dark stumps. The upper back teeth may follow. A child may avoid cold drinks, chew on one side, or be irritable at mealtimes, though many toddlers show no pain until decay is advanced. Bad breath, swelling on the gum or a pimple-like bump suggest infection and need prompt review.

Why night feeding causes it

Milk, formula and juice all contain sugars that mouth bacteria turn into acid. During sleep, saliva flow drops, so sugary liquid pools around the teeth for hours with little natural rinsing. Falling asleep with a bottle, frequent on-demand night feeds after teeth erupt, sweetened drinks in a sippy cup throughout the day, and dipping pacifiers in honey or syrup all raise the risk. Bacteria can also pass from a carer's mouth through shared spoons. Breastfeeding is valuable and is not in itself the problem; the risk rises when frequent night feeding is combined with other sugars and no brushing after teeth appear.

How it is treated

Treatment depends on how far the decay has progressed. White spot lesions can often be stopped or partly reversed with fluoride varnish applied at the clinic, a fluoride toothpaste routine at home and changes to feeding habits. Small cavities may be repaired with tooth-coloured fillings. Larger cavities in baby molars may need a pediatric crown, and decay reaching the nerve may need a pulpotomy. Badly broken teeth with infection sometimes have to be removed. The aim is to keep the child comfortable and keep baby teeth in place for chewing, speech and holding space for adult teeth. Visits are kept short and paced to the child.

Prevention for babies and toddlers

Start cleaning as soon as the first tooth appears, using a soft infant brush and a smear of fluoride toothpaste the size of a grain of rice, twice a day. Brush last thing at night after the final feed. Avoid putting your child to bed with a bottle of milk or juice; if a bottle is needed for comfort, plain water is safest. Aim to move from bottles to an open or free-flow cup around the first birthday. Keep juice and sweet drinks out of the daily routine, and avoid sweetening pacifiers. Lift the upper lip once a month to look for white lines along the gum.

When to book a visit

A first dental visit is recommended by the first birthday or within six months of the first tooth. Book sooner if you see white bands, brown patches or chipped front teeth, if your child seems to have pain when eating, or if there is a history of decay in older siblings. Seek same-day care for facial swelling, swelling around the eye, fever with tooth pain, or a child who will not eat or drink; facial swelling with fever or a very unwell child needs urgent medical assessment. Early visits are also an opportunity for parents to learn brushing positions and ask about feeding without judgement.

Positive early visits shape a child's attitude to dentistry for life. We keep appointments short, explain everything in child-friendly language, and focus on prevention.

Questions patients ask

Patients visit us from Al Safa, Al Wasl, Jumeirah, Umm Suqeim, Al Quoz, City Walk, Business Bay, Downtown Dubai and Al Barsha — and from Sharjah and the wider UAE.

Is breastfeeding at night bad for teeth?

Breastfeeding has many benefits. Risk increases when frequent night feeds continue after teeth erupt alongside other sugars and without brushing, so cleaning teeth after the last feed matters.

Can bottle decay be reversed?

Very early white spots can often be stopped and partly remineralised with fluoride and habit changes. Once a cavity forms, it usually needs treatment.

Do decayed baby teeth matter if they will fall out?

Yes. Decay can cause pain and infection, affect eating and sleep, and early loss of baby teeth can lead to crowding of adult teeth.

Is fluoride toothpaste safe for babies?

A rice-grain smear of fluoride toothpaste is recommended from the first tooth and is considered safe at that amount.

What should my baby drink at bedtime?

After teeth appear, plain water is the safest drink in a bedtime bottle or cup.

What does baby bottle tooth decay look like and where can I get my toddler checked in Dubai?

Baby bottle tooth decay usually starts as chalky white lines along the gums of the upper front teeth and can turn brown or chip away; Dr. Fariba Alnoory examines toddlers for this at Manhal Medical Center in Dubai. Early white spots can often be managed with fluoride varnish and changes to feeding.

My 2-year-old has brown front teeth from night feeding — what treatment will they need?

At Manhal Medical Center in Dubai, Dr. Fariba Alnoory assesses how deep the decay is before advising treatment. Options range from fluoride and small tooth-coloured fillings to crowns or nerve treatment for larger cavities, with visits kept short and gentle for young children.

How can I stop my baby getting tooth decay from milk at night?

Dr. Fariba Alnoory at Manhal Medical Center, Dubai, advises brushing with a rice-grain smear of fluoride toothpaste after the last feed and avoiding bedtime bottles of milk or juice once teeth appear. Plain water is the safest night-time drink, and a first dental check by age one helps catch early signs.

Is it urgent if my toddler's decayed tooth causes swelling?

Yes — Dr. Fariba Alnoory at Manhal Medical Center in Dubai advises that facial swelling from a decayed tooth needs same-day dental care, and that swelling with fever or a very unwell child needs urgent medical assessment. Parents should call the clinic as soon as swelling appears.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.
