Pain after tooth extraction — normal timeline, severe pain after days and pain relief
Some discomfort after an extraction is expected, but people often worry when pain is still there after a few days or comes back later. Understanding the normal pattern helps you recognise when pain is part of healing and when it needs attention.

Pain after a tooth extraction is usually strongest in the first 24 to 72 hours and then steadily improves over about a week; pain that gets worse after day two or three, or returns weeks later, often points to dry socket, infection or a retained fragment and should be checked. At Manhal Medical Center in Dubai, Dr. Moataz Al-Khan and Dr. Manhal Helfawee assess post-extraction pain and treat the cause.
Key facts
- Pain peaks in the first 1–3 days and eases over a week
- Worsening pain at day 2–4 often means dry socket
- Dry socket is treatable with a soothing dressing
- Aspirin is generally avoided after extractions
- Spreading swelling or fever needs urgent care
The normal pain timeline
Once the anaesthetic wears off, a dull ache and tenderness around the socket are normal. Pain usually peaks within the first one to three days, particularly after surgical or wisdom tooth removal, when swelling is also at its greatest. From around day three, pain should gradually ease each day, and by about a week most people need little or no pain relief. After a simple extraction of a loose tooth, discomfort may be mild and last only a day or two. Removing an inflamed or infected tooth may hurt a little more afterwards, but removing the source of infection often brings relief overall.
Severe pain after two days or a week: dry socket
If pain improves and then becomes much worse around day two to four, dry socket (alveolar osteitis) is a common cause. The blood clot breaks down or is dislodged, leaving bone exposed. Pain is typically intense, throbbing and may radiate to the ear, temple or neck, often with a bad taste or smell, and the socket may look empty. Dry socket is more common after lower wisdom tooth removal and in smokers. It is not usually dangerous, but it is very uncomfortable. Treatment involves gently cleaning the socket and placing a soothing dressing, which often eases pain quickly.
Other causes of persistent pain
Pain that persists or begins later can have other causes. Infection may cause increasing swelling, pus, fever, a foul taste or swollen glands. A small bone spicule or tooth fragment working its way out can cause a sharp, localised sore spot. Jaw stiffness and muscle pain are common after long procedures on lower wisdom teeth. Occasionally, pain is actually coming from a neighbouring tooth or sinus. Pain at the extraction site two months later is not typical and should be assessed with an examination and X-ray, as it may relate to a fragment, infection or a nearby problem.
Pain relief after an extraction
Follow the advice given by your surgeon. For many healthy adults, regular doses of an anti-inflammatory such as ibuprofen, sometimes alternated or combined with paracetamol, are commonly recommended for dental pain, provided these medicines are safe for you. Taking them on schedule for the first two to three days often works better than waiting for pain to build. Aspirin is generally avoided after extractions because it can increase bleeding. People with stomach ulcers, kidney disease, asthma sensitive to anti-inflammatories, pregnancy or those on blood thinners should ask their doctor or pharmacist. Cold packs on the cheek in the first day help with swelling and discomfort.
When to call — and post-extraction care at Manhal in Dubai
Contact the clinic if pain is getting worse after day two or three, if painkillers no longer control it, if you notice a bad taste or smell, or if swelling increases after day three. Seek urgent care for spreading facial or neck swelling, difficulty swallowing or breathing, high fever or trouble opening your mouth. At Manhal Medical Center in Dubai, Dr. Moataz Al-Khan, specialist oral and maxillofacial surgeon, and Dr. Manhal Helfawee, consultant implantologist, see patients with post-extraction pain, diagnose the cause and treat dry socket, infection or fragments. Extractions are performed under local anaesthesia, with written aftercare.
When a tooth can't be saved, careful extraction protects the surrounding bone for future replacement. We discuss implant or bridge options before the tooth is removed.

Like many people, I don't enjoy the dentist. For many years I avoided visits out of fear. I found Manhal 15 years ago, and he is a wonderful dentist. Calming presence. Excellent implantology as well as other dental work. I have recommended him personally to many friends, and will continue to do so. I just hope he doesn't ever retire. When I have been overseas with dental problems, I have flown back to Dubai specifically to see him. Could not imagine ever going to another dentist.this was my first visit to Dr Manhal after recomendation from my husband, i needed his consultation after i visited more than one doctor in Lebanon with no luck, i am glad i visited him, he is very professional the clinic is very clean with great staffAmazing service and very friendly team I'm very happy with my dental implant results with Dr.Manhal ❤️
